약독화된 박테리아가 종양 조직에 특이적으로 군집되어 성장하기 때문에 박테리아 암 치료법은 새로운 종양 치료법으로 대두대고 있으며, 유전자 발현 시스템을 인위적으로 설계하는 합성 생물학 기술은 치료 유전자를 종양 특이적으로 발현할 수 있도록 하고 있다. 본 연구에서 ppGpp 생합성이 결손되어 Salmonella typhymurium 약독화 균주(△ppGpp)가 L-arabinose(L-ara) 존재 시 인터루킨 15(IL15)와 플라겔린 B(FlaB)의 융합 단백질인 IL15/FlaB를 발현하도록 설계된 신규 균주 SLpIF를 제작하였다. L-ara 가 첨가된 시험관 배지에서 배양된 SLpIF에서 FlaB와 IL15의 각각의 생체활성을 모두 가지는 IL15/FlaB를 세균 외로 분비하였다. 여러 마우스 종양 모델에 정맥 투여시, SLpIF는 L-ara가 생체 내 투여되었을 때 FlaB 혹은 IL15만을 분비하는 박테리아 치료 보다 더 높은 종양 억제 효능와 증가된 생존율을 보였다. SLpIF로 치료된 마우스의 종양 대식세포는 항염증성 M2 유사 표현형에서 염증성 M1 유사 표현형으로 현저한 변화가 관찰되었고, 종양 조직 내 CD8+ T 및 NKT 세포에서 더 높은 증식과 활성화가 관찰되었다. 또한, SLpIF 접종으로 종양이 치료된 마우스의 종양세포 재접종 시 50% 이상에서 종양이 생기지 않았다. 이는 이들 마우스에서 종양에 대한 면역성이 유지되고 있음을 보여준다. 치료가 어렵고 높은 전이율을 보이는 4T1 및 B16F10 종양 마우스에서 SLpIF의 치료효과는 면역 관문 억제제인 항PD-L1 항체의 동시 투여 시 더욱 향상되었다. 이들 마우스에서 복합치료는 박테리아 단독 치료 보다 좀더 향상된 종양 치료효과와 증가된 생존율을 보여주었다. 결론적으로, 향상된 치료효능을 가진 박테리아의 제조 및 면역관문억제제와의 동시 투여 치료는 박테리아 암 면역치료법의 임상 적용에 새로운 방법론이 될 것이다.
The use of synthetic biology to design expression systems for therapeutic gene(s) in attenuated bacteria is an appealing approach to tumor therapy because the bacteria specifically target tumor tissue. Here, we engineered an attenuated Salmonella typhimurium deficient in ppGpp biosynthesis (△ppGpp) to express IL15/FlaB, a fusion protein comprising interleukin-15 (IL15) and flagellin B (FlaB), in the presence of an inducer (L-ara), named SLpIF. In vitro experiments showed that IL15/FlaB secreted from bacteria retained the bioactivity of both FlaB and IL15. We then assessed the antitumor effects of the engineered bacteria in several murine tumor models. In the presence of L-ara, SLpIF showed more efficient inhibition of tumor growth and increased mouse survival, to a greater extent than bacteria expressing either FlaB or IL15 alone. In SLpIF-treated mice, tumor macrophages showed a pronounced shift from an M2-like to an M1-like phenotype, accompanied by increased proliferation and activation of tumor-infiltrating CD8+ T, NK, and NKT cells. Moreover, we observed robust immune memory against rechallenged tumor cells in ≥50% SLpIF vaccinated mice. The therapeutic effects of SLpIF in mice bearing highly malignant 4T1 and B16F10 tumors were enhanced by combination with an anti-PD-L1 antibody, an immune checkpoint inhibitor. Combined treatment increased tumor suppression and prolonged survival. Taken together, these data provide new insight into the design and efficacy of bacterial-mediated cancer immunotherapy.
