목적: 본 연구에서는 폐암 정위체부방사선치료 (4D-SBRT) 시 사차원전산화단층촬영 (4DCT) 기반 O-ring 선형가속기의 고속치료의 임상 활용 가능성을 확인하고자 한다.대상 및 방법: 고려대학교 안암병원에서 치료받은 38 명의 폐암 4D-SBRT 환자들을 대상으로 하였다. 대상환자는 전 호흡주기의 평균 호흡 진폭이 0.51 cm 이하인 환자들을 대상으로 하였다. 4DCT 영상은 전 호흡주기를 10 개의 phase로 구별하여 획득하였고, 50% phase를 기준으로 종양과 장기를 설정하였다. Gating20-70% plans은 C-arm 선형가속기를 이용한 phase 20-70%의 사차원전산화단층촬영의 데이터를 기반으로 치료받은 환자의 방사선치료계획 데이터를 획득하였다. Non-GatingFB plans은 O-ring 선형가속기를 이용하여 phase 0-90%의 사차원전산화단층촬영 기반으로 후향적으로 치료계획을 수립하였다. 처방 선량은 계획용표적체적 (PTV)에 총 4,800 cGy를 4 분획으로 전달하도록 처방하였다. Gating20-70%과 Non-GatingFB plans의 선량학적 차이를 비교하기 위해, PTV에 대해서는 conformity index (CI), homogeneity index (HI), gradient measure (GM)을 분석하였다. 손상위험장기 (OARs)에 대해서는 Radiation Therapy Oncology Group (RTOG) 0915 protocol을 이용하여 비교 분석하였다. Purpose: In this study, we aimed to verify the feasibility of fast four-dimensional computed tomography (4DCT) based O-ring linear accelerator (LINAC) treatment in lung 4D-stereotactic body radiotherapy (4D-SBRT). Materials and Methods: This study included data of 38 patients who received lung 4D-SBRT at Korea University Anam Hospital. Patients with average respiratory amplitude < 0.51 cm in the full phase were included in the study. The 4DCT images were obtained by dividing the entire respiratory cycle into 10 phases, and tumors and organs were set based on the 50% phase. Gating20-70% plans were based on 4DCT data obtained at phase values ranging from 20–70% using a C-arm LINAC. Non-GatingFB plans were retrospectively established based on 4DCT data obtained at phase values of 0–90% using an O-ring LINAC.
