In the present study, the author examined whether aqueous extract of YLT, a complex herbal extract, empirically used in traditional Asian medicine for controlling diverse pulmonary diseases,affects cigarette smoke-induced pulmonary pathology in a rat model of inflammatory pulmonary disease. Rats were exposed by inhalation to a mixture of cigarette smoke extract (CSE) and sulfur dioxide for three weeks to induce pulmonary inflammation. YLT was administered orally to rats and pathological changes to the pulmonary system were examined in each group of animals through measurement of tumor necrosis factor- α (TNF- α) and interleukin-6 (IL-6) levels in bronchoalveolar lavage fluid (BALF) at 21 days post-CSE treatment. The effect of YLT on the hypersecretion of pulmonary mucin in rats was assessed by quantification of the amount of mucus secreted. In order to develop a novel candidate for controlling the production and/or secretion of airway mucus from YLT, an effective complex herbal extract, betulin, lupeol, oleanolic acid, resveratrol, and kaempferol, as the active components of YLT, were isolated and purified. Effect of these natural products was examined on the gene expression and production of MUC5AC mucin and epidermal growth factor receptor (EGFR)-mitogen-activated protein kinase (MAPK) and nuclear factor kappa B (NF-kB) signaling pathway, in human airway epithelial cells. Confluent NCI-H292 cells were pretreated with betulin, lupeol, oleanolic acid, resveratrol, and kaempferol for 30 min and then stimulated with epidermal growth factor (EGF) or phorbol 12- myristate 13-acetate (PMA), for the following 24 h. The effect on PMA-induced NF-kB signaling pathway or EGF-induced MAPK signaling pathway was also investigated. The MUC5AC mucin gene expression and production of mucin protein were measured by RT-PCR and ELISA, respectively. Effect of betulin, lupeol, oleanolic acid, resveratrol, and kaempferol on EGFR- MAPK and NF-kB signaling pathway was checked by western blot analysis. YLT inhibited CSE-induced pulmonary inflammation as shown by decreased TNF-α and IL-6 levels in BALF. YLT inhibited the hypersecretion of in vivo pulmonary mucin and MUC5AC mucin production in vitro. Betulin, lupeol, oleanolic acid, resveratrol, and kaempferol suppressed the production and gene expression of MUC5AC mucin, induced by PMA through the inhibition of degradation of inhibitory kappa Bα (IkBα), and NF-kB p65 nuclear translocation. Betulin, lupeol, oleanolic acid, and resveratrol did not affect the production and gene expression of MUC5AC mucin, induced by EGF. However, among the five natural products, kaempferol inhibited the production and gene expression of MUC5AC mucin and suppressed the phosphorylation of EGFR, phosphorylation of p38 MAPK and extracellular signal-regulated kinase (ERK) 1/2 (p44/42), and the nuclear expression of specificity protein-1 (Sp1). In summary, betulin, lupeol, oleanolic acid, resveratrol, and kaempferol can regula the gene expression and production of airway mucin and, at least partly, this finding explains the folk use of YLT as anti-inflammatory folk remedy in various inflammatory pulmonary diseases. Also, these five compounds can be developed as a therapeutic agent controlling hypersecretion of airway mucus via regulation of NF-kB and/or MAPK signaling pathway, through further investigation.
본 연구에서는 다양한 호흡기 염증성 질환에서 관찰되는 점액 과다 분비를 조절할 목적으로 민간에서 사용되어 왔던 복합 생약 추출물인 YLT와, 그 구성 생약인 화피, 사삼, 산수유, 호장근, 황정에서 유래하는 단일 천연물인 betulin, lupeol, oleanolic acid, resveratrol, kaempferol 등 5개 의 천연물을 대상으로 기도 상피세포에서의 MUC5AC 뮤신 유전자의 발현, 뮤신 생성, 세포 내 MAPK 와 NF-kB 신호전달 경로에 대한 효과 를 검증함으로써 호흡기 염증성 질환에서 관찰되는 기도 점액의 과다 생성 및 분비를 조절해 줄 수 있는 신약후보 물질을 발굴하고자 하였 다. 담배연기 추출물과 이산화황 혼합 흡입으로 유발된 호흡기 염증성 점액(뮤신) 과다분비 동물모델에 생약 복합추출물인 YLT를 3주간 경구 투여한 후, 기관-폐포 세척액으로의 뮤신 분비, 종양괴사인자-α 및 인터 루킨-6의 생성 및 분비에 대한 약물의 영향을 효소연계 면역흡착 분석 법 (ELISA) 을 통하여 평가하였다. In vitro 모델로서 뮤신의 생성 및 유 전자 발현과 관련된 세포 신호전달 경로 연구에 주로 사용되는 인체 기도 상피세포주인 NCI-H292 세포를 대상으로, 상피세포 성장인자인 EGF와 phorbol ester인 PMA에 의해 각각 유도된 MUC5AC 뮤신의 생성 및 MUC5AC mRNA 발현에 대한 betulin, lupeol, oleanolic acid, resveratrol, kaempferol 등 5종 천연물의 영향을 각각 ELISA 및 역전사 중합효소 연 쇄반응법 (RT-PCR) 을 통하여 검증하였다. 이러한 천연물들의 분자약리학적 작용기전을 규명하기 위하여, 뮤신 생성을 매개하는 신호전달 경로인 EGFR-MAPK와 NF-kB 신호전달 경로에 미치는 영향을 western blotting을 이용하여 검증하였다. 본 연구의 결과는 다음과 같았다. (1) YLT는 동물실험 모델과 in vitro 세포 모델에서 기도 뮤신의 분비를 용 량의존적으로 억제하였다. (2) YLT는 동물실험 모델에서 염증유발성 사 이토카인인 종양괴사인자-α 와 인터루킨-6 분비를 용량의존적으로 억제 하였다. (3) YLT를 구성하는 5개 생약의 주요 활성성분인 betulin, lupeol, oleanolic acid, resveratrol, kaempferol 등 5개 천연물은 각각 PMA로 유도 된 뮤신 유전자의 발현 및 뮤신의 생성을 억제하였다. (4) 5개 천연물 중 betulin, lupeol, oleanolic acid, resveratrol은 공히 EGF로 유도된 뮤신 유 전자의 발현 및 뮤신의 생성에는 유의한 영향을 주지 못하였다. 그러나, kaempferol은 EGF로 유도된 뮤신 유전자의 발현 및 뮤신의 생성을 억 제 하 였 다 . (5) betulin, lupeol, oleanolic acid, resveratrol, kaempferol 은 공 히PMA에 의해 야기되는 IkBα의 phosphorylation 및 degradation과 NF-kB p65의 phosphorylation 및 translocation을 억제함으로써, 최종적으로 MAC5AC 뮤신 유전자의 transcription을 억제할 가능성을 보여주었다. (6) 5개 천연물 중 kaempferol은 EGF에 의해 촉발되는 EGFR의 phosphorylation, p38의 phosphorylation, ERK1/2의 phosphorylation과 Sp1의 핵 내 발현을 억제함으로써, 최종적으로 MAC5AC 뮤신 유전자의 transcription을 억제할 가능성을 보여주었다. 이상의 연구결과를 요약하 면, betulin, lupeol, oleanolic acid, resveratrol, kaempferol 등의 천연물은 기도 뮤신을 생성, 분비하는 배상세포에 직접 작용하여, 뮤신의 유전자 발현 및 뮤신의 생성 단계에서 NF-kB 그리고/또는 EGFR-MAPK 신호전달 경 로를 경유하여 조절 또는 억제작용을 발현함으로써, 기도 점액 과다 생 성 및 분비를 효과적으로 조절할 수 있는 신약후보물질로 개발될 가능 성을 일부 제시하고 있다.
