레이노 현상은 저온과민성 질병이며 저온에 노출되었을 때 손과 발의 혈관들에 혈류가 감소하는 특징을 보인다. 이러한 레이노 현상을 치료하기 위해서 다양한 전통한약들이 오랫동안 사용되어져 왔다. 대표적인 한약으로 당귀사역가오수유생강탕,당귀, 우차신기환이 있으며 이 약재들은 중국, 일본, 한국에서 많이 사용되어지고있다. 다양한 임상연구를 통해 이 약재들의 효능이 검증되었지만, 레이노현상을 치료하는 작용 메커니즘이 아직까지 명확하지 않다. 따라서, 본 연구에서는 당귀사역가오수유생강탕, 당귀, 우차신기환의 레이노현상 치료 효능에 대한 비임상 연구를통해 작용기전과 효능을 검증하고자하였다. 당귀사역가오수유생강탕을 쥐에게 경구투여를 통해 투여한 결과, 저온에 의해 수축된 꼬리 혈관이 상대적으로 이완됨을확인했다. 또한 당귀사역가오수유생강탕의 치료효과는 저온에 의해 활성화된 RhoA단백질 관련 기작을 억제하여 나타나는 것임을 확인했다. 당귀와 우차신기환을 혈관내피세포와 혈관주변세포에 처리하면 저온에 의해 활성화된 RhoA 단백질의 발현을 감소시켰고 혈관내피세포의 수축기작 또한 억제했다. 그리고, 우차신기환을 혈관내피세포에 처리한 결과 저온에 의한 endothelin-1 증가와 nitric oxide 감소를 억제했다. 결론적으로, 본 연구를 통해 전통 한약인 당귀사역가오수유생강탕, 당귀,우차신기환의 레이노현상 치료 효능과 작용 메커니즘을 규명했다
Raynaud’s phenomenon is characterized with cold hypersensitivity and reduced blood flow in cutaneous capillaries and arterioles at hands or feet under cold condition. Traditional herbal medicinal products have been used to treat Raynaud’s phenomenon. Danggui-Sayuk-Ga-Osuyu-Senggang-Tang, angelica gigas and Ucha-Shinki-Hwan have long been prescribed to manage Raynaud’s phenomenon in China, Japan and South Korea. While efficacy of those herbal medicinal products has been demonstrated through clinical studies, preclinical studies for elucidating the biological mechanism on the treatment of Raynaud’s phenomenon are further needed. Therefore, the present study was designed to report the preclinical evidence for therapeutic efficacy of traditional herbal medicinal products for treatment of Raynaud’s Phenomenon. Danggui-Sayuk-Ga-Osuyu-Senggang-Tang suppressed vasoconstriction in cold-exposed mouse tail vein via inhibiting ras homolog gene family member A-related contractile pathway. Furthermore, angelica gigas and Ucha-Shinki-Hwan also mitigated cold-mediated contraction of vascular cells via inhibiting the activation of ras homolog gene family member A. In addition, Ucha-Shinki-Hwan reversed cold-mediated increase of endothelin-1 and decrease of nitric oxide in cultured medium of human dermal microvascular endothelial cells. Thus, this study demonstrated that traditional herbal medicinal products could be a promising alternative medicine for treatment of Raynaud’s phenomenon.
