A 4-element millimeter wave multiple-input multiple-output (MIMO) antenna is presented for future fifth generation (5G) wireless communication systems. Slotted rectangular patch antenna with slot-based ground plane has been used as monopole elements in the MIMO design. The designed antenna operates in dual bands covering frequency range 27.30-30.25 GHz (10.25%) and 36.19-40.27 GHz (10.67%). Isolation of more than 28 dB and 17 dB is realized between all the ports for the resonating bands just by employing polarization diversity and no decoupling structure has been used. Thus, the designed antenna has simple structure and common ground plane is used for all the antennas. The suggested antenna is designed on top of a $14\text{mm}\times 14\text{mm}\times 0.79\text{mm}$ low loss Rogers RT5880 substrate. The simulated gain of MIMO antenna is observed to be 7.09 dBi and 3.43 dBi in the two operating bands. The diversity gain and Envelope correlation coefficient are found within the admissible limits, thus resulting in good MIMO diversity performance. The proposed antenna is highly eligible to be used for 5G communication devices.
