서론 : 2019년 정부는 노인인구가 증가함에 따라 의료와 복지를 아우르는 통합적인 서비스를 시행하도록 하여 통합돌봄 사업과 재가의료급여시범사업을 시행하였다. 의료급여수급자들을 대상으로 한 재가의료급여시범사업은 장기입원 하던 의료급여수급자들이 퇴원 이후에 집에 돌아가서도 의료, 돌봄, 이동, 식사에 대한 서비스를 필요시 통합적으로 받을 수 있도록 하는 사업이며 의료급여관리사들이 핵심적 역할을 하고 있다. 이에 시범사업에 대한 효과평가를 위해 만족도와 재입원 의사에 대한 평가가 필요한 시점이다. 연구목적 : 본 연구에서는 첫째, 시범사업 참여자들의 시범사업에 대한 서비스 경험과, 둘째, 이용자의 경험에 따른 시범사업 만족도차이 및 만족도 영향요인, 셋째, 재입원 의도에 따른 시범사업에 대한 경험의 차이를 확인하고 이를 통해 사업의 확대 및 지속가능성을 위한 제언을 하고자 한다. 연구방법 : 전국 13개 시범사업 지자체의 참여대상자 116명에게 2020년 5월 14일부터 5월 28일까지 2주간 전화설문을 실시하였다. 사전에 구조화된 설문지를 구성하여 시범사업 서비스 만족도, 재입원의도, 장기입원사유, 서비스이용경험, 사회적 관계 등을 질문하였다. 주요결과 : 시범사업 참여대상자 116명 중 103명(88.8%)이 사업에 전반적으로 만족한다고 응답하였다. 이중 가장 만족하는 서비스는 돌봄이었고 식사서비스와 주거개선이 뒤를 이었다. 응답자 중 92명(79.3%)이 재입원을 고려하지 않고 있다고 응답하여 시범사업의 효과성을 입증하였다. 의료급여관리사에 대한 만족도가 이용자의 사업전반에 대한 만족도에 영향을 미치는 요인으로 나타났으며, 재입원의도에 따라 삶의질, 의료급여관리사에 대한 만족도와 사업전반에 관한 만족도에 차이가 있는 것으로 나타났다. 결론 : 만족도 조사결과 재가의료급여시범사업을 이용한 대상자들은 지역사회연계서비스에 대부분 만족하였으며, 재입원을 고려하고 있지 않아 사업이 목적한 바에 따라 성과를 달성하였다고 볼 수 있다. 더불어 연구결과를 토대로 사업의 확장과 지속성을 고려했을 때, 의료급여관리사의 친절도를 높이고 사업 전반에 대한 만족도와 삶의 질을 높여 재입원의도를 감소시킨다면 성공적인 Aging in Place를 실현할 수 있을 것이다.
Objectives : The effectiveness of the medical aid community care program was analyzed using the experience, satisfaction, and intention to hospital readmission of medical aid recipients. Methods : The total number of participants was 116, and they were living in 13 areas. We surveyed from May 15th to 28th. We asked satisfaction with the services, degree of intention to readmission, the reason for long-term hospital admission, experience regarding service utilization, frequency of social contacts before and after hospital discharge. Results : Overall, 103 people out of 116 (88.8%) responded that they were satisfied with the project. Ninety-two respondents(79.3%) answered that they do not consider hospital readmission, proving that the pilot project is practical. Satisfaction was different between people who used mobility, meal, personal care assistant and those who do not. Also, there were differences between those who want hospital readmission and those who do not want to. We found the satisfaction with the case manager to be a factor affecting the overall service satisfaction of users. Conclusion : Most of the subjects were satisfied with the services and did not consider re-hospitalization. So the project achieved promising results according to the purpose. Increasing the satisfaction of the case manager can cause increased overall satisfaction. Also, overall satisfaction and satisfaction with the case manager, increasing quality of life can be a strategy to extend the length of residence in their community.
