연구목적 : 코르티코스테로이드는 강력한 항염증성 약물로 널리 사용되고 있다. 이러한 광범위한 사용에도 불구하고, 코르티 코스테로이드 현재사용자와 고용량사용자에서 심혈관계질환 발생가능성이 여러 연구에서 제기되어 왔다. 한국인에서 코르티코 스테로이드의 사용과 급성심근경색증 발생위험에 대한 인과적 관련성을 평가하고자 후향적 코호트연구를 수행하였다. 연구방법 : 연구 자료는 2009년 건강보험심사평가원 입원환자표본자료를 사용하였다. 코호트의 정의는 2009년 4월부터 9월까지 한 가지 성분의 코르티코스테로이드만을 처방받은 20세 이상 환자이었다. 추적관찰기간 중 급성심근경색증 발생시점, 표본자료 종료시점, 사망한 시점 가운데 먼저 관찰된 것을 중도절단으로 정의하였다. 병용약물, 동반질환 등의 교란변수들을 보정하여 콕스의 비례위험모형을 이용하여 상대위험도 및 95% 신뢰구간을 산출하였다. 연구결과 : 총 104,801명 중 737명의 급성심근경색증 발생 환례를 확인하였고 평균발생률은 1,000 인년당 14명으로 나타났다. 프레드니솔론 사용자에 비해 메칠프레드니솔론의 급성심근경색증 발생할 상대위험도는 1.26(95% 신뢰구간=1.01-1.57) 로 통계적으로 유의하게 높았다. 프레드니솔론-용량환산기준 평균하루용량을 분석한 결과, 고용량 사용자에서의 급성심근경색증 발생할 상대위험도는 11.65(95% CI:8.18-16.60)로 통계적으로 유의하게 높았고, 남성, 여성, 40-64세, 65세 이상, 류마티스 관절염, 천식/만성폐쇄성폐질환, 관상동맥질환 환자, NSAIDs 선택성이 높은 약물 복용 환자, 항응고제 복용 환자 유무에 관계 없이 고용량 사용자에서 높았다. 결론 : 프레드니솔론에 비하여 메칠프레드니솔론에서 급성심근경색증 발생 위험증가를 보였고, 코르티코스테로이드 사용량이 증가할수록 위험도가 증가함을 파악하였다. 코티코스테로이드 성분별 급성심근경색증 발생위험을 비교하기 위하여 추후 더 많은 환자를 포함하는 장기간 데이터를 활용한 연구를 수행할 필요가 있다. 또한, 코르티코스테로이드 고용량사용자에서의 급성심근경색증 발생 위험증가를 고려하여 주의 깊은 모니터링이 필요하다.
Objectives : Corticosteroids are widely prescribed potent anti-inflammatory drugs. Several reports raised the risk of cardiovascular diseases associated to the current use and high dose of corticosteroids. We performed a retrospective cohort analysis to evaluate the risk of acute myocardial infarction (AMI) associated with the use of corticosteroids in Korea. Methods : We used the Korea Health Insurance Review and Assessment Service-National Inpatients Sample (HIRA-NIS) database. The study was conducted on 20 years or older who prescribed corticosteroids between April 2009 and September 2009. Corticosteroids users were followed until they were diagnosed with acute myocardial infarction, until death, or until the end of the study. The Cox proportional hazards model was used to calculate hazard ratios (HRs) and their 95% confidence intervals (CIs), while adjusting for comedications and comorbidities. Results : AMI was reported in 737 of 104,801 corticosteroid users (14 per 1,000 person-years). Compared with prednisolone users, the HR of AMI was 1.26 (95% CI=1.01-1.57) in methylprednisolone users. High dose user of prednisolone-equivalent mean daily dose was associated with an increased risk of AMI (adjusted HR 11.65, 95% CI=8.18-16.60), which was consistent among male, female, aged 40-64, 65 over, and patients with or without rheumatoid arthritis, asthma/chronic obstructive pulmonary disease, coronary heart disease, NSAIDs, and anticoagulants. Conclusion : Based on our study results, the risk of AMI increased in methylprednisolone users compared with prednisolone users. Further study needs to perform using for long-term data and high dose users of corticosteroids needs careful monitoring for AMI occurrence.
