본 연구에서는 뇌졸중 환자의 보행 보조 도구 사용에 따른 과제 지향적 보행 능력과 보행 속도 및 기 능적 수행 능력에 미치는 효과를 알아보고자 하였다. 참가자들은 국립 재활원에 뇌졸중 입원환자 57명을 대상으로 하였다. 실험 전 후 결과를 측정하기 위해 보행 능력(10m walk test, 10mWT), 과제 지향 평가 (modified Emory Functional Ambulation Profile, mEFAP, Chedoke-McMaster Stroke Walking Assessment, CMSWA), 하지 근력(Motricity Index, MI), 하지 운동 기능(Fugl Meyer-Lower Extremity, FM-LE), 근경직(Modified Ashworth Scale, MAS) 측정, 그리고 균형(Berg Balance Scale, BBS)을 사용 하였다. 3개의 보조 도구 그룹의 각각의 차이는 걷기 능력에 대한 예후 인자를 식별하기 위해 수행 된 일원분산분석(One-way ANOVA)과 단계적 다중회귀 분석(Stepwise Multiple regression analysis)을 사용하여 평가 하였다. 사후 분석하여, 통계적으로 유의한 차이가 mEFAP(F=30.67, p=.001), CMSWA(F=18.23, p=.001), MI(F=11.64, p=.001), FM-LE(F=8.39, p=.001), 그리고 BBS(F=16.57, p=.001)에 있었다. 10m 보행 검사에서 독립 보행군과 지팡이군이 4점 지팡이군에 비해 유의 한 차이가 있었다. 독립 보행군이 4점 지팡이군에 비해 MI- 슬관절 신전근과 MAS- 발목 경직에서 유의한 차 이가 있다. 독립 보행군이 두 그룹에 비해 MI- 족관절 배측 굴곡근에서 유의한 차이가 있다. 독립 보행군과 지팡이 군이 4점 지팡이군에 비해 MI- 고관절 굴곡근에서 유의한 차이가 있다. 회귀 분석은 BBS와 보행 보 조기구가 mEFAP(R2=.66)의 가장 중요한 독립적인 결정 요인이라고 밝혔다. CMSWA와 10m 보행검사에서 는 BBS, MI-총합, 보행 보조 도구 순으로 나타났다(R2=.57, R2=.43). 이러한 연구 결과는 뇌졸중 후 개개인 의 보행 도구를 사용한 사람이 도움을 받지 않고 걷는 사람보다 보행 능력, FM-LE, BBS가 좋지 않다. 본 연 구의 결과는 뇌졸중 후 개개인의 보행 보조 도구의 처방을 신중하게 물리치료사에 의해 고려되어야 함을 시사하고, 특히 재활의 초점은 균형 및 운동 기능의 회복을 포함한다.
The present study was to investigate the effects of two different walking aids on task-oriented walking capacity, gait velocity, and functional performance ability. The subjects were 57 stroke inpatients from National Rehabilitation Center. Walking capacity was measured by 10m Walking Test, modified Emory Functional Ambulation Propile, Chedoke-McMaster Stroke Walking Assessment, and Motor function of lower extremity was measured by Morticity Index, Fugl Meyer-Lower Extremity, Modified Ashworth Scale-ankle spasticity. Berg Balance Scale was used to evaluate the balance ability. Difference between each of the 3 assistive devices group were assessed using the one-way ANOVA and multiple stepwise regression analyses were performed to identify prognostic factors for Walking capacity. By post hoc analysis, statistically significant differences showed up in mEFAP(F=30.67, p=.001), CMSWA(F=18.23, p=.001), MI-total(F=11.64, p=.001), FM-L/E(F=8.39, p=.001) and BBS(F=16.57, p=.001). No-aid and 1-point groups had statistically significant differences 4-point cane group in the 10 m WT. No-aid group had statistically significant differences 4-point cane group in the MI-knee extensor and MAS-ankle spasticity. No-aid group hadstatistically significant differences two groups in the MI-ankle dorsiflexor. No-aid and 1-point stick groups had statistically significant differences 4-point cane group in the MI-hip flexor. Regression analyses revealed that the BBS and walking aids were the most important independent determinant of the mEFAP(R2=.66). The BBS, MI-total and walking aids were the most important independent determinants of the CMSWA and 10m WT, respectively(R2=.57, R2=.43). These findings indicate that post-stroke individuals who used a walking aids had poor walking capacity, motor function of lower extremity, balance than counterparts who walked unaided. The outcome of this study suggests that prescription of a walking aids for a post-stroke individuals should be carefully considered by the physiotherapist, especially if the focus of rehabilitation includes restoration of balance and motor function.
