본 연구는 재난적의료비가 의료비에 의해서만 발생하는 것이 아니라 건강악화에 의한 가구근로소득 상실에 의해 영향 받을 수도 있음을 확인하고자 하였다. 이 연구는 한국복지패널 10차 자료를 이용하여 중장기 입원이 근로소득 감소 매개효과를 통해 재난적의료비 발생에 미치는 영향을 살펴보았다. 분석 결과는 첫째, 중장기 입원은 그 자체로 재난적의료비 발생에 정(+)의 영향을 미쳤으며, 둘째, 근로소득에 부(-)의 영향을 미쳤다. 셋째, 중장기 입원이 재난적의료비 발생에 영향을 미치는데 있어 근로소득이 매개하였다. 결과적으로 중장기 입원이 근로소 득의 감소를 야기하고, 이것이 재난적의료비 측정을 왜곡할 수 있음을 확인하였다. 재난적의료 비는 가구소득 대비 의료비의 비율로서 정의되는 지표임에도 불구하고 선행연구들의 문제의식은 의료비에만 초점이 맞춰져왔다. 재난적의료비가 가지고 있는 특성은 심각한 건강악화를 내포하고 있는 바, 이 건강악화가 가계의 소득에 영향을 미칠 수 있다는 사실도 인지해야한다. 정책적으로는 의료보장성 확대의 관점을 소득보장의 관점으로까지 확대하여야 한다.
The main focus of this study is to show that Catastrophic Health Expenditure (CHE) is not caused only by high copayments, but also the income reduced by the deterioration of health. This study examined the impact of mid-to-long-term hospitalization (MLTH) on the incidence of catastrophic medical expenses through the mediating effect on the reduction of earnings. The results of the analysis were as follows: First, the MLTH itself had a positive impact on the occurrence of CHE. Second, MLTH had a negative effect on earned income. Third, earned income was mediated by the effects of MLTH on CHE outbreaks. Although CHE is an indicator defined as the ratio between household income and health care costs, the critical minds of previous studies have focused on health care costs only. It is also important to note that the characteristics of CHE have serious health consequences, which can affect household incomes. As a policy, the view of expanding medical security should be extended to that of income security.
