본 연구의 목적은 코로나19 시대 노인요양시설의 보건·복지서비스 전달체계 업무 경험을 파악하여 위기 상황을 효과적으로 극복하는 전달체계 방안을 모색하는 데 있다. 구체적인 업무 경험을 파악하기 위해 노인요양시설이 코로나19 감염자 발생으로 코호트 격리가 이루어졌을 때 보건소, 시·군·구, 국민건강보험공단 간 전달체계 업무를 어떻게 수행하는지 분석하였다. 일반적 질적연구에 근거하여 코로나19 감염자 발생으로 코호트 격리된 노인요양시설을 중심으로 노인 보건·복지서비스 전달체계 업무를 경험한 실무자 총 13명(노인요양시설 8명, 시·군·구 2명, 보건소 2명, 국민건강보험공단 1명)을 개별심층인터뷰하였다. 연구 결과, 노인 보건·복지서비스 전달체계 업무 경험에 대해 32개의 개념, 10개의 하위범주, 3개의 범주가 발견되었으며, 3개의 범주는 ‘역할을 찾아가는 기관들’, ‘불통을 넘어서 새로운 소통창구 마련’, ‘관리·지원 없이 자체 대응하는 노인요양시설’로 나타났다. 연구결과를 바탕으로 첫째, 기관들의 감염병 대응을 위한 역할 구분 및 시·군·구의 컨트롤 타워 역할, 둘째, 감염병 대응지침 교육 및 감염병 발생 시 Hot Line 개설, 셋째, 노인요양시설의 의료서비스 지원 내실화 및 정신건강 및 후유증 관리 지원 필요성을 실천적, 정책적 함의로 제시하였다.
The purpose of this study is to find a delivery system plan to effectively overcome crisis situations by grasping the work experience of the health and welfare service delivery system of nursing homes in the COVID-19 era. To understand the specific work experience, this study analyzed what kind of health and welfare service delivery system tasks was performed between nursing homes, public health centers, local governments, and the National Health Insurance Service (NHIS) when cohort isolation was conducted due to COVID-19. Based on a general qualitative study, in-depth interviews were conducted with thirteen working-level workers(eight nursing home workers, two local governments worker, two public health center workers, and one NHIS worker) who related to nursing home’s delivery systems task. As a result, a total of 32 concepts, 10 sub-themes, and 3 main themes were conducted for the work experience of the nursing home’s health and welfare service delivery system. The main themes included 'Each organization looking for accountability', 'Reaching new channels of communication beyond communication difficulties', and 'A nursing home responds COVID-19 to itself without management or support'. This study suggests that the task of the nursing home delivery system need to be changed in practical ways, such as giving local governments a role as control towers, developing guidelines and training programs, providing New Hot Lines, and supporting the expansion of medical and health care services in nursing homes.
