배경: 기저세포암은 가장 흔한 피부암의 하나로 알려져 있으며, 특히 아시아인에서는 대부분의 기저세포암에 색소가 침착되어 있다. 색소가 있는 기저세포암의 경우 비색소성에 비해 수술적 경계가 작으며 좀 더 높은 완전제거율을 보이는 것으로 보고된 바 있다. 이전 연구에서는 기저세포암의 색소도가 높을수록 종양의 악성도와 반비례한다는 보고가 된 바 있다.목적: 본 연구에서는 기저세포암 환자에서 더모스코피 검사와 조직병리학적 패턴을 분석하여 색소도를 정량적으로 측정하고, 종양의 악성도와의 연관성을 살펴보았다. 이를 통해 기저세포암의 색소도가 예후에 좋은 인자로 작용함을 확인하고자 하였다.연구대상 및 방법: 총 76명의 모즈 미세도식수술을 시행 받은 환자를 대상으로 연구를 진행하였으며, 종양의 침윤 깊이와 모즈 미세도식수술 병기를 종양의 악성도 지표로 설정하였다. 파라핀 고정된 종양 병리표본을 Fontana Masson 특수염색을 시행하여 멜라닌 색소를 측정하였으며, Melan-A 및 HMB-45 면역염색을 시행하여 멜라닌세포와 멜라닌탐식세포의 면적을 이미지 측정 프로그램을 통해 분석하였다.결과: 더모스코피 상 측정된 색소도는 종양의 침윤 깊이와 모즈 미세도식수술 병기와 역의 상관관계를 보였으며, 조직병리학적 색소도와는 양의 상관관계를 보였다. 멜라닌 색소, 멜라닌세포 및 멜라닌탐식세포의 상대 면적 측정값은 종양의 악성도와 역의 상관관계를 보이는 유의미한 결과를 나타내었다. 시행한 로지스틱 회귀분석에서도 조직병리학적 색소도가 낮을수록 기저세포암이 중간층 및 하층부에 침윤할 가능성이 유의미하게 높았다.결론: 본 연구에서는 기저세포암의 색소도가 종양의 악성도와 유의미한 역의 상관관계가 있음을 정량적인 분석을 통해 밝혀내었고, 이것이 기저세포암의 좋은 예후 인자가 됨을 입증하였다. 임상의사들은 더모스코피 검사를 통해 기저세포암의 색소도를 측정하여 이를 바탕으로 기저세포암의 침윤 깊이를 예측할 수 있을 것으로 생각되며, 추후 수술적 계획을 설정하는데 있어서도 도움이 될 것으로 판단된다.중심어: 기저세포암, 모즈 미세도식수술
Background: Basal cell carcinoma (BCC) is the most common skin cancer; in Asian population, most BCCs are pigmented. It is reported that pigmented BCC requires a small surgical margin and shows a more complete resection rate than non-pigmented BCC. Previous studies have suggested that increased pigmentation in BCCs may be inversely associated with tumor aggressiveness in Asian patients. Objective: In this study, we analyzed the dermoscopic features and histopathologic patterns of patients with BCC to evaluate the quantitative correlation between the pigmentation of BCC and tumor aggressiveness. We aimed to confirm whether pigmentation in BCC can be considered a favorable prognostic feature.Materials and Methods: A total of 76 patients with BCC lesions who had clinical pigmentation were included. Mohs micrographic surgery (MMS) stage and tumor depth were measured as indices of tumor aggressiveness. Paraffin-blocked tumor specimens were stained with Fontana Masson for identification of melanin, immunohistochemical analysis were performed using Melan-A/MART-1 and HMB-45/Melanosome to identify melanocyte or melanophage. The relative areas of dermoscopic pigmentation, melanin and melanocyte/melanophage stained area were measured using computerized image analysis software (Image Pro Plus V.3.0).Result: Dermoscopic pigmentation showed a significant inverse correlation with tumor infiltration depth and MMS stage in Spearman correlation analysis. The melanin area and melanocyte-stained area showed a significant inverse correlation with tumor infiltration depth and MMS stage. In MMS stage 1, the mean dermoscopic pigmentation value was 34.48%(± 14.22). For MMS stages 2 and 3, the mean dermoscopic pigmentation values were 13.72%(± 7.54) and 15.50%(± 17.52), respectively. In addition, in the logistic regression model, higher dermoscopic pigmentation(OR=0.82, 95% CI 0.68-0.99), melanin(OR=0.75, 95% CI 0.63-0.89), and melanocyte-stained areas(OR=0.80, 95% CI 0.70-0.92) were associated with a lower possibility of BCC tumor infiltration over the middle layer (ML) and lower layer (LL).Conclusion: We found an inverse correlation between the pigmentation and aggressiveness of BCCs using a novel quantitative method. By conducting dermoscopy tests, clinicians can predict the subclinical infiltration depth of BCC based on the pigmentation value. We suggest that pigmentation can be considered as a favorable prognostic factor for BCC.Keywords: Basal cell carcinoma, Mohs micrographic surgery
