우리나라의 노인장기요양정책은 가능한 오랫동안 살던 곳에서 노후를 보내고자 하는 노인의 Aging in Place(AIP) 욕구를 반영한 재가급여 우선 제공을 원칙으로 하고 있다. 그러나 노인의 AIP 욕구가 좌절되었을 경우 삶의 만족도에 어떠한 영향을 미치는가에 대한 이론적, 실증적 근거는 찾아보기 어렵다. 따라서, 본 논문은 살던 곳을 떠나 AIP 욕구를 실현할 수 없는 노인장기요양시설 거주노인들의 삶의 만족도에 주목하였다. 본 연구는 주로 지역사회 거주노인에게 적용되어 왔던 개념인 성공적 노화 요인 중 시설 거주노인 상황에 적용할 수 있는 주관적 건강상태, 주관적 경제상태, 그리고 가족·동료 입소자·직원 등 사회적지지와 시설 만족도 및 시설 입소 전 AIP 욕구를 독립변수로 사용하여 이들이 시설 거주노인의 삶의 만족도에 미치는 영향을 탐색하였다. 아울러 시설 거주노인의 입소 전 AIP 욕구 정도에 따라 이들 영향요인이 삶의 만족도에 미치는 영향 간에 차이가 있는지를 살펴보았다. 이론적 고찰과 선행연구 검토를 통해 시설 거주노인의 성공적 노화 수준이 높을수록, 시설만족도가 높을수록 삶의 만족도가 높을 것이고, 입소 전 AIP 욕구가 높을수록 삶의 만족도는 낮을 것이라는 가설을 설정하였다. 또한, 시설 거주노인의 AIP 욕구 정도를 설문 응답을 바탕으로 고·저집단으로 구분하고 이들 집단 간에 성공적 노화요인과 시설만족도가 삶의 만족도에 미치는 영향력에 차이가 있을 것이라는 가설을 설정하였다. 가설검증을 위하여 시설 거주노인의 건강 특성과 코로나바이러스감염증 19 유행상황을 감안하여 시설의 종사자가 인지능력이 있는 시설 거주노인에게 설문을 읽어주고 응답을 받아적는 방법으로 설문을 실시하였다. 설문조사에 협조한 47개 시설에서 수집된 433명의 자료를 분석하였으며, 결과는 다음과 같다. 첫째, 시설 거주노인의 성공적 노화요인인 주관적 건강상태와 주관적 경제상태가 좋을수록, 시설 거주노인의 가족, 동료 입소자, 시설직원으로부터의 사회적지지가 높을수록 삶의 만족도가 통계적으로 유의하게 높게 나타났다. 둘째, 시설 거주노인의 시설만족도가 높을수록 삶의 만족도는 높게 나타났다. 셋째, 시설 거주노인의 입소 전 AIP 욕구가 높을수록 삶의 만족도가 낮게 나타났으나 두 변수간 상관관계가 통계적으로 유의하지 않아 가설은 기각되었다. 넷째, AIP 욕구 고집단에서는 모든 독립변수가 삶의 만족도에 통계적으로 유의미한 정(+)의 영향을 미치는 반면, AIP 욕구 저집단에서는 사회적지지만이 삶의 만족도에 유의미한 정(+)의 영향을 미치는 것으로 나타났다. 그러나, AIP 욕구 고·저 집단간 영향력 차이를 알아보기 위한 기술통계 평균 비교(t-test), 상관계수 비교(Fishers’ Z-test), 회귀계수 비교(Chow-test) 검정 결과 모두 통계적으로 유의미한 차이가 나타나지 않았다. 이에 본 연구는 다음과 같은 학문적, 정책적 함의를 제시하였다. 우선, AIP 욕구를 장기요양정책에서 중요한 개념으로 반영하기 위해서는 이론적, 실증적 근거를 위한 추가연구가 필요하다. 둘째, 주로 재가노인을 대상으로 적용하였던 성공적 노화 이론이 시설 거주노인의 삶의 만족도에도 영향을 미치는 요인으로 나타났으므로 시설 거주노인의 삶의 만족도를 높이기 위한 요인들에 대한 적극적인 학문적 연구 및 실천적 정책방안 마련이 필요하다. 셋째, 노인의 AIP 욕구는 존중하되 재가정책을 우선시하고 시설정책을 주변화하기보다는 노인의 노화상태에 따라 재가급여와 시설급여가 연속적인 차원으로 추진되어야 한다. 이를 위해 재가와 시설생활의 장단점을 상호 보완한 주거 또는 시설형태를 모색할 필요가 있다. 넷째, 시설 거주노인의 AIP 욕구를 보완·대체할 수 있도록 시설환경 및 서비스 개별화, 시설 내 교류활동 촉진, 시설 직원의 전문성 강화 등을 추진할 필요가 있다.
In principle, Korea's long-term care policy for the elderly reflects the elderly's desire to spend their old age in the place where they have lived for as long as possible(Aging in Place: AIP), and provides home care preferentially. However, it is difficult to find theoretical and empirical studies on effect on the life satisfaction of the elderly when AIP desire is frustrated. Therefore, this paper paid attention to the life satisfaction of the elderly living in long-term care facilities who left the place where they lived. To this end, successful aging factors such as subjective health status, subjective economic status, and social support from family, fellow residents, and employees of facilities which mainly have applied to the elderly living in the community but can be applied to the elderly living in the facility too, facility satisfaction, and AIP desire before entering the facility were used as independent variables and investigated effect on life satisfaction used as dependent variables. In addition, this paper looked at the difference in the effect of these influencing factors on life satisfaction depending on the degree of AIP desire before admission of the elderly living in the facility. Through theoretical consideration and review of previous studies, the hypothesis was established that the higher the level of successful aging and the higher the facility satisfaction, the higher the life satisfaction of the elderly living in the facility, and the higher the AIP desire before admission, the lower the life satisfaction. In addition, based on the survey responses of the elderly, the AIP desire was classified into high and low groups, and the hypothesis was established that there would be a difference in the influence of successful aging factors and facility satisfaction on life satisfaction. In order to verify the hypothesis, considering the characteristics of the elderly with poor physical and mental health conditions and the COVID-19 epidemic, the survey was conducted by the facility workers reading the survey and writing down responses to the elderly with cognitive abilities, and the 433 data collected from 47 facilities that cooperated with the survey were analyzed. The analysis results are as follows. First, it was found that the better the subjective health status, subjective economic status, and the social support from the family, fellow residents, and facility employees which are the successful aging factors of the elderly living in the facility used in this paper, the higher the life satisfaction. Second, the higher the facility satisfaction of the elderly living in the facility, the higher the life satisfaction. Third, it was found that the higher the AIP desire of the elderly living in the facility before admission, the lower the life satisfaction, but the hypothesis was rejected because the correlation between the two variables was not statistically significant. Fourth, in the AIP desire stubborn group, all independent variables had a statistically significant positive(+) effect on life satisfaction. On the other hand, in the AIP needs low group, only social support was found to have a significant positive(+) effect on life satisfaction. However, there was no statistically significant difference in the comparative analysis of the descriptive statistical mean(t-test), correlation coefficient(Fishers' test), and regression coefficient(Chow-test) to determine the difference in influence between AIP desire high and low groups. Therefore, this study presented the following academic and policy implications. First of all, additional research for theoretical and empirical evidence is needed to reflect AIP desire as an important concept in long-term care policies. Second, since the successful aging theory, which was mainly applied to the elderly at home, was found to be a factor that affects the life satisfaction of the elderly living in the facility, active academic research and practical policy measures are needed. Third, while respecting the AIP desire of the elderly, home care and facility benefits should be promoted in a continuous dimension according to the aging state of the elderly rather than prioritizing home care policies and marginalizing facility policies. To this end, it is necessary to find a housing or facility type that complements the advantages and disadvantages of living in a house and facility. Fourth, it is necessary to promote individualization of facility environment and services, promotion of exchange activities in facilities, and strengthening the expertise of facility employees so that AIP desire of the elderly living in facilities can be supplemented and replaced.
