본 연구의 목적은 G2SFCA 모형을 기반으로 한 개선된 응급 의료 서비스의 공간적 접근성 측정 방법을 제시하고, 이를 청주시에 적용하여 정책적 시사점을 도출하는 것이다. 개선된 응급 의료 서비스 접근성은 급성 심장정지 발생률로 추정된 수요를 이용하면서 서로 다른 골든타임 특성을 반영한 출동 접근성과 이송 접근성의 합으로 측정된다. 급성 심장정지 발생률을 고려하지 않는 기존의 응급 의료 접근성과 비교할 때, 급성 심장정지 환자의 응급 의료 접근성은 일반적인 응급 의료 접근성과 유사한 패턴을 보이지만 일반적인 응급의료접근성에 비해 농촌 지역에서 접근성이 더 열악한 것으로 나타나 이 지역에 접근성 개선 정책이 시급히 추진될 필요가 있음을 파악할 수 있다. 연구결과를 토대로 이 연구는 급성 심장정지 환자의 생존률 향상을 위해 새로운 시설 공급뿐만 아니라 119 지역대의 추가 설치를 통한 출동 접근성 향상, 출동 접근성이 낮은 지역의 자동심장충격기 우선 설치를 제안한다.
This study aims to present an G2FCA-based improved method for measuring the spatial accessibility of emergency medical services, and to suggest policy implications by applying it to Cheongju City. The comprehensive accessibility to emergency medical services suggested in this study consists of access to emergency medical services and access to hospitals, which have different distance-decay function based on different golden-time characteristics by medical services. In particular, we estimated the demand for sudden out-of-hospital cardiac arrest (SOHCA) in each area of Cheongju City based on its incidence rate. SOHCA patients access to emergency medical services shows a similar geographic pattern as the general population. However, in rural areas, these patients have poorer access to emergency medical services compared to the general population, indicating that policies to improve access are urgently needed in these areas. Based on the findings, this study recommends improving access to emergency medical service through the provision of new facilities as well as additional 119 branch firehouse, and prioritizing the installation of AEDs in areas with low access to emergency medical care in order to improve the survival rate of SOHCA.
