A 79-year-old woman had been suffering from nausea and vomiting over 10 years. Abdominal CT showed a superior mesenteric vein (SMV) rotation sign, whirl-like pattern around the superior mesenteric artery (SMA) and no evidence of mesenteric ischemia. We diagnosed intestinal malrotation with midgut volvulus and laparoscopic repair of the volvulus and Ladd procedure was performed. No intraoperative or postoperative complications were noted, and the patient was discharged from the hospital on the 16th postoperative day. Laparoscopic Ladd procedure is a safe and feasible technique for intestinal malrotation with midgut volvulus in adult cases.
