目的 完善在温州医科大学附属第二医院(简称该院)进行住院医师规范化培训(简称住培)的西藏自治区(简称西藏)全科住院医师质量评价体系,提升全科住培质量和全科医师岗位胜任力,以便更好地适应当地医疗需求.方法 构建全科住院医师岗位胜任力评价指标体系,以我院 2016 级—2018 级共 24 名西藏全科住培结业住院医师为研究对象,通过问卷调查结业住院医师的岗位胜任力自评和继续教育需求,将收回的数据进行描述性分析.结果 本研究显示,结业住院医师均回藏工作,其中 60.9%住院医师目前从事全科专业,对自身岗位胜任力总体评分良好,其中临床基本技能自我评价最高,临床基础诊疗能力、急危重症识别处理能力及医学人文素养评价次之,公共卫生服务能力及慢性病管理能力评分较低,岗位提升能力评分最低.83.0%结业住院医师选择支持西藏住院医师赴浙江等沿海地区完成住培培训.结论 西藏住院医师对该院住培的总体认可度高,住院医师结业后均能够胜任现在的临床岗位.建议在西藏全科住培方案中适当增加急危重症识别能力、公共卫生服务能力和慢性病管理能力的培训内容,并在结业住院医师继续教育中增加岗位提升能力的内容.
Objective To improve the quality evaluation system of standardized training of general practitioners from Tibet,the quality of residency training and the competency of general practitioners,so as to enable the general practitioners to meet the local medical needs.Methods This paper constructed a general practitioner post competency evaluation index system.A total of 24 general practitioners from Tibet who had completed assessment in this hospital from 2019 to 2021 were taken as the research subjects.Through questionnaire survey,post competency self-assessment and continuing education demand survey of them were carried out,and descriptive analysis was made of the collected data.Results The study showed that all the general practitioners were to work in Tibet,and 60.9%of them were currently engaged in general practice.The overall score of post competency of the general practitioners was good,and the self-evaluation of basic clinical skills of the residents was the highest,followed by the evaluation of basic clinical diagnosis and treatment ability,emergency critical illness identification and management ability,and medical humanistic literacy.The scores of public health service ability and chronic disease management ability were low,and the score of post promotion ability was the lowest.83%of the general practitioners chose to support Tibet residents to complete residency training in coastal areas such as Zhejiang Province.Conclusion General practitioners from Tibet have a high overall recognition of the standardized residency training system in this hospital,and they are qualified for the current clinical posts after graduation.It is suggested that the training content of emergency critical illness recognition ability,public health service ability and chronic disease management ability should be appropriately added to the training program of general practitioners,and the content of post improvement ability should be added to the continuing education of general practitioners after graduation.
