目的 探讨血清脂联素(APN)、脑源性神经营养因子(BDNF)与短暂性脑缺血发作患者认知功能的关系.方法 回顾性分析无锡市惠山区人民医院 2020 年 1 月~2023 年1 月收治的108 例短暂性脑缺血发作患者的临床资料,根据认知功能分为认知功能障碍组(n=48)和认知功能正常组(n=60).通过受试者工作特征曲线(ROC)分析血清APN、BDNF预测短暂性脑缺血发作患者发生认知功能障碍的价值;通过多因素logistic回归性分析明确短暂性脑缺血发作患者发生认知功能障碍的危险因素.结果 认知功能障碍组受教育年限≤6 年、高血压、糖尿病、静息性腔梗的患者占比显著高于认知功能正常组,血清APN、BDNF水平显著低于认知功能正常组,差异有统计学意义(P<0.05);经过ROC分析,血清APN、BDNF预测短暂性脑缺血发作患者发生认知功能障碍的曲线下面积分别为 0.731、0.728(P<0.05);经多因素logistic回归性分析,受教育年限≤6 年、高血压、糖尿病、静息性腔梗、APN≤6.120 μg∕ml、BDNF≤4.980 ng∕ml是短暂性脑缺血发作患者发生认知功能障碍的危险因素(P<0.05).结论 血清APN、BDNF与短暂性脑缺血发作患者认知功能显著相关,血清APN、BDNF水平降低预示短暂性脑缺血发作患者存在认知功能损害.
Objective To investigate the relationship between serum adiponectin(APN),brain-derived neurotrophic factor(BDNF)and cognitive function in patients with transient ischemic attack.Methods The clinical data of 108 patients with transient ischemic attack admitted to our hospital from January 2020 to January 2023 were retrospectively analyzed,and they were divided into cognitive dysfunction group(n=48)and normal cognitive function group(n=60)according to cognitive function.The value of serum APN and BDNF in predicting cognitive dysfunction in patients with transient ischemic attack was analyzed by receiver operating characteristic curve(ROC).Multivariate logistic regression analysis was used to determine the risk factors of cognitive dysfunction in patients with transient ischemic attack.Results The proportion of patients with years of education≤6 years,hypertension,diabetes and resting cavity infarction in cognitive dysfunction group was significantly higher than that in normal cognitive function group,and the serum APN and BDNF levels were significantly lower than those in normal cognitive function group(P<0.05).After ROC analysis,the areas under the curve of serum APN and BDNF to predict cognitive dysfunction in transient ischemic attack patients were 0.731 and 0.728(P<0.05),respectively.Multivariate logistic regression analysis showed that years of schooling≤6 years,hypertension,diabetes,resting infarction,APN≤6.120 μg∕ml,BDNF≤4.980 ng∕ml were risk factors for cognitive dysfunction in patients with transient ischemic attack(P<0.05).Conclusion Serum APN and BDNF are significantly correlated with cognitive function in patients with TIA,and lower levels of APN and BDNF predicted cognitive impairment in patients with TIA.
