Giriş ve Amaç: Akne vulgaris multifaktöriyel sebeplerin tetikleyici olarak rol aldığı, pilosebase ünitenin kronik inflamatuar bir hastalığıdır. Hastalık patogenezinden sorumlu faktörler; anormal foliküler keratinizasyon, aşırı sebum üretimi, P.acnes kolonizasyonu ve inflamasyondur. Mikro komedonlarda inflamatuar belirteçlerin bulunduğu immünohistokimya yoluyla da gösterilmiştir. Nötrofillerin de bu inflamatuar sürece katkısı aşikârdır. Akne Vulgaris tedavisinde sık kullanılan doksisiklin ve isotretinoin, antiinflamatuar etkileri ortaya konan iki tedavi ajanıdır. Biz de bu çalışmada İsotretinoin ve Doksisiklin tedavilerinin Nötrofil Lenfosit Oranı ' na etkisini karşılaştırmayı amaçladık.Gereç ve Yöntem: Atatürk Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Deri ve Zührevi Hastalıklar polikliniğine başvuran akne vulgaris hastalarından; 18–40 yaşAralığındaki 100 hasta çalışmaya alındı. Çalışmamıza enfeksiyon veya sistemik inflamatuar hastalığı olan kişiler dahil edilmemiştir. Bu hastalardan 50'si için doksisiklin; diğer 50'si için isotretinoin tedavisi planlandı. Kontrol grubuna enfeksiyon öyküsü ve sistemik inflamatuar bir hastalığı olmayan ve herhangi bir ilaç kullanmayan, 50 sağlıklı birey alındı. Çalışma grubunun tedavi öncesi ve tedavi verildikten sonraki 3.aydaki ve kontrol grubunun NLO (nötrofil lenfosit oranı) değerleri kayıt edildi. Hasta grubunun Global Akne Skorlama Sistemi'ne göre akne şiddeti hesaplandı. Çalışma sonucu elde edilen verilerin istatistiksel analizleri için SPSS (Statistical Package for Social Sciences) Version 22 programı kullanıldı (Mann Whitney U ve Wilcoxon Testleri uygulanmıştır.)Bulgular: Yaptığımız istatistiki analizde, Nötrofil/Lenfosit oranı düzeyini hasta grubunda kontrol grubuna göre anlamlı derecede yüksek saptadık. Ayrıca Nötrofil/Lenfosit oranı düzeyini isotretinoin ve doksisiklin planlanan hastayüksek saptadık. Bunun ardından hastalık şiddeti ile Nötrofil/Lenfosit oranı arası ilişki olup olmadığını inceledik ve akne skoru ile Nötrofil/Lenfosit oranı arasında zayıf bir pozitif ilişki tespit ettik. Nötrofil/Lenfosit oranı düzeyleri arasındaki anlamlı fark yalnızca aknesi olmayan grup ile orta şidette aknesi olan gruplar arasında bulundu. İsotretinoin tedavisi planlanan hastaların, tedavi öncesi ve tedavinin 3.ayında Nötrofil/Lenfosit oranı düzeylerine bakıldığında; tedavinin 3. ayında Nötrofil/Lenfosit oranı düzeylerinde istatiksel olarak anlamlı azalma saptanmadı. Doksisiklin tedavisi planlanan akne vulgaris hastalarının tedavi öncesi ve tedavinin 3.ayında NLO değerleri karşılaştırıldığında tedavinin 3.ayında Nötrofil/Lenfosit oranı düzeylerinde istatiksel olarak anlamlı azalma tespit edilmiştir.Sonuç: İnflamatuvar hastalıklarda arttığı gösterilen ve prognostik öneme sahip olduğuna dair çalışmalar bulunan Nötrofil/Lenfosit oranı ile akne vulgaris arasındaki ilişkiyi göstermeyi amaçladığımız çalışmamızda Akne Vulgaris hastalarında anlamlı Nötrofil/Lenfosit oranı düzeyi yüksekliği saptadık. Ayrıca antiinflamatuar etkinlikleri bilinen iki ilaç olan doksisiklin ve isotretinoin tedavilerinin Nötrofil/Lenfosit oranı üzerine etkisini inceledik. Tedavi öncesi ve sonrasındaki 3.ay Nötrofil/Lenfosit oranı değerlerini karşılaştırdığımızda doksisiklin tedavisinin Nötrofil/Lenfosit oranı düzeyini istatiksel olarak anlamlı ölçüde azalttığını, isotretinoin tedavisinde Nötrofil/Lenfosit oranında istatiksel olarak anlamlı ölçüde değişiklik olmadığını tespit ettik. Literatürde akne şiddeti Nötrofil/Lenfosit oranı ilişkisini ve doksisiklin ve isotretinoin tedavilerinin Nötrofil/Lenfosit oranı üzerine etkilerini karşılaştıran çalışma mevcut değildi. Yapılacak daha büyük çalışmalarla akne vulgaris hastalarının hastalık şiddeti tespiti ve tedavi takibinde Nötrofil/Lenfosit oranı düzeyinin tercih edilebilecek bir belirteç olabileceği kanaatindeyiz. Introduction and Purpose: Acne vulgaris is a chronic inflammatory disease of the pilosebaceous unit that multifactorial causes play a role. Factors which are responsible for disease pathogenesis are abnormal follicular keratinization, excessive sebum production, P.acnes colonization and inflammation. The presence of inflammatory markers in micro comedones has also been demonstrated by immunohistochemistry. The contribution of neutrophils to this inflammatory process is obvious. Doxycycline and isotretinoin, which are commonly used in the treatment of acne vulgaris, are two therapeutic agents with anti-inflammatory effects. In this study, we aimed to compare the effect of isotretinoin and doxycycline treatments on neutrophil lymphocyte ratio.Materials and Methods: 100 acne vulgaris patients being in the age range of 18-40 who applied to the Skin and Venereal Diseases Polyclinic of Atatürk University Medical Faculty Research and Application Hospital were included to the study. In our study, people with infection or systemic inflammatory disease were not included in the study. Doxycycline treatment was planned for 50 of these patients and isotretinoin treatment was plannedfor the other 50 patients. 50 individuals were included to the control group who had no history of infection and systemic inflammatory disease and who did not use any medications. The NLO (neutrophil-lymphocyte ratio) values of the control group and the NLO values of the study group before treatment and at the third month after treatment were recorded. Acne severity was calculated according to the Global Acne Scoring System of the patient group. SPSS (Statistical Package for Social Sciences) Version 22 program was used for statistical analysis of the data obtained from the study.Results: In our statistical analysis, Neutrophil / Lymphocyte ratio was significantly higher in the patient group compared to the control group. In addition, when neutrophil / lymphocyte ratio was examined separately in isotretinoin anddoxycycline patient groups, we found that the levels were significantly higher than the control group. We then examined if there is a relationship between the severity of the disease and the neutrophil / lymphocyte ratio and found a weak positive correlation between the acne score and the neutrophil / lymphocyte ratio. Significant difference between neutrophil / lymphocyte ratio levels was found only between the group without acne and the groups with moderate acne. When the levels of neutrophil / lymphocyte ratio of patients receiving isotretinoin treatment were evaluated before and at the third month of treatment, it is seen that; Neutrophil / Lymphocyte ratio was not significantly decreased statistically at 3 months of treatment. When the NLO values of acne vulgaris patients who were planned to be treated with doxycycline were compared to pre-treatment and at the third month of treatment, a statistically significant decrease in the neutrophil / lymphocyte ratio wasfound in the third month of treatment.Result: We aimed to show the relationship between neutrophil / lymphocyte ratio and acne vulgaris, which have been shown to increase in inflammatory diseases and have studies that have prognostic significance and in our study, we found a significant high level of Neutrophil / Lymphocyte ratio in Acne Vulgaris patients. We also examined the effects of doxycycline and isotretinoin treatments, two drugs with known anti-inflammatory activity, on the Neutrophil / Lymphocyte ratio.When we compared Neutrophil / Lymphocyte ratio values before and after the treatment, we found that doxycycline treatment significantly decreased the Neutrophil / Lymphocyte ratio level and that Neutrophil / Lymphocyte ratio did not change significantly in isotretinoin treatment.In the literature, there was no study comparing the relationship between acne severity and neutrophil / lymphocyte ratio and the effects of doxycycline and isotretinoin treatments on neutrophil / lymphocyte ratio. With far-reaching studies we believe that the level of Neutrophil / Lymphocyte ratio may be a preferable marker in disease severity detection and treatment follow-up of patients with acne vulgaris 81
