The article describes the case of a 34-year-old female who have controlled hypertension and asthma, Marfan's syndrome and a replacement mechanical aortic heart valve. Her case highlights the complication relating to the perioperative management of patients with mechanical heart valves taking oral vitamin K antagonists. Podiatric surgeons are advised to have sufficient knowledge and understanding to manage these high-risk patients appropriately.
