A 52-year-old female presented in March 2017 with several months of intermittent headache and a malodorous scalp lesion (Fig.). Coronal computed tomography scan (Fig. a) and magnetic resonance imaging (Fig. b) revealed complete erosion of the overlying calvarium and areas of cortical/subcortical brain invasion. Brain was then covered with synthetic, suturable dural substitute and then pre-shaped titanium mesh immediately overlying (Fig. a) followed by a latissimus dorsi free flap (Fig. b).
