Discuss the five themes identified regarding the role of palliative care in the use of advanced therapies for heart failure patients: educating patients on disease state, prognosis, and treatment options; understanding patient goals of care; managing symptoms; addressing psychosocial barriers; and managing end-of-life care Advanced therapies for heart failure (mechanical circulatory support and transplant) can improve quality of life and prolong survival, but these treatment options demand appropriate decision making with the patient and may require overcoming barriers to improve patient eligibility. This study investigated provider perspectives on the role of palliative care in the treatment of heart failure patients with advanced therapies. This qualitative study took place at an academic medical center with an integrated cardiac palliative care inpatient team and outpatient clinic. Semistructured interviews were conducted with cardiologists, cardiac surgeons, and nurse practitioners who provide care for heart failure patients. Interview transcripts were analyzed deductively and inductively to reveal themes surrounding the role of palliative care in the treatment of patients with advanced therapies. Many providers expressed that the integration of palliative care into the process of caring for patients who were considering or undergoing treatment with advanced therapies was both expected and valuable. Five themes were identified regarding the role of palliative care in the use of advanced therapies for heart failure patients: educating patients on disease state, prognosis, and treatment options; understanding patient goals of care; managing symptoms; addressing psychosocial barriers; and managing end-of-life care. Cardiology providers recognize the value of palliative care across multiple palliative domains to support heart failure patients who are considering and undergoing treatment with advanced therapies. Integration of palliative care into advanced therapy teams can improve provider understanding of the longitudinal, multidimensional role of palliative care in advanced heart disease and break down traditional barriers to palliative care integration in heart failure. This study will help shift the perception of palliative care among cardiology providers nationally by displaying the perceptions of referring providers at an academic medical center with a mature palliative care program.
