He was taken up for total thyroidectomy with central compartment lymph node dissection with bilateral selective lymph node dissection. The non-recurrent laryngeal nerve (NRLN) is an extremely rare anatomical variation reported to be present in 0.4-0.6% on the right side and 0.004% on the left side.1 Damage to the recurrent laryngeal nerves is one of the dreaded complications of thyroid and parathyroid surgery.2 In case of NRLN this risk is further increased. 4 Donatini G, Carnaille B, Dionigi G.
