EVIDENCE-BASED ANSWER: Abnormal monofilament testing has positive likelihood ratios >7 and negative likelihood ratios ranging from 0.07 to 0.61 for diagnosing neuropathy in patients with diabetes. (SOR: B, systematic review of heterogeneous cohort studies and single cohort study). Absent monofilament sensation is sufficient to diagnose diabetic peripheral neuropathy (SOR: C, expert opinion).
