Intra-abdominal gas insufflation used during laparoscopic surgery causes adverse circulatory and respiratory effects. This can be hazardous for patients with compromised cardiac or pulmonary function or both, and there is an increased awareness of the intra-operative risks during laparoscopy. Recent studies indicate that there is a potential for pharmacological treatment of circulatory abnormalities during laparoscopic surgery, possibly enabling even compromised patients to benefit from the postoperative advantages of the technique. The recent development of methods for gasless laparoscopy also reflects the urge for less cardiopulmonary side effects of the laparoscopic technique.
