Purpose: This study was conducted to determine the operational effect of the rapid response team by comparing the incidence of cardiac arrest and the clinical outcomes of CPR in patients in the ward before and after the activities of the first group rapid response team.Methods: A total of 200 subjects were patients over the age of 18 who were admitted to the general ward of C Certified tertiary hospital in G city from January 1, 2018 to February 28, 2021, and 113 patients were admitted before the activities of the rapid response team from January 1, 2018 to July 31, 2019, and 87 patients were admitted after the activities of the rapid response team from August 1, 2019 to February 28, 2021. Based on the rapid response system screening program records, electronic medical records and in-hospital CPR records, the incidence of cardiac arrest, the subject's CPR-related characteristics, and the clinical results after CPR by time were investigated. The collected data were analyzed by applying a two-sided test at a significance level of 0.05 using SPSS/WIN 25.0. The general characteristics of the study subjects were analyzed as descriptive statistics, and the continuous variables were analyzed as mean and standard deviation, and the categorical variables were analyzed as frequencies and percentages. Differences in general characteristics, cardiopulmonary resuscitation-related characteristics, and post-CPR clinical outcomes and CPR clinical outcomes by time were analyzed by the Independent T-test, Mann-Whitney U test, Chi-square test, or Fisher's exact test. The ICU survival rate according to the presence or absence of rapid response team activity was identified by Kaplan-Meier Survival Analysis. The homogeneity of the survival curves for each group was confirmed using a log-rank test.Results: The main findings of this study are as follows.1.The number of cardiac arrests per 1,000 hospitalized patients in the ward after rapid response team activities was 1.90, a significant decrease from 2.39 cases per 1,000 hospitalized patients in the ward before rapid response team activities (p
연구목적: 본 연구는 1군 신속대응팀 활동 전 ž 후 병동 환자의 심정지 발생률 및 심폐소생술 임상결과를 비교하여 신속대응팀의 운영 효과를 파악하기 위해 수행되었다.연구방법: 연구대상자는 총 200명으로 2018년 1월 1일에서 2021년 2월 28일까 지 G시에 소재한 C 상급종합병원의 일반병동에 입원한 만 18세 이상의 환자 중 심폐소생술을 시행 받은 자이며, 2018년 1월 1일부터 2019년 7월 31일까지 신 속대응팀 활동 전 시기 113명, 2019년 8월 1일부터 2021년 2월 28일까지 신속 대응팀 활동 후 시기 87명이다. 신속대응시스템 스크리닝 프로그램 기록, 전자의 무기록 및 원내 심폐소생술 기록지를 토대로 심정지 발생률, 대상자의 심폐소생 술 관련 특성 및 심폐소생술 후 임상결과, 시간대별 심폐소생술 후 임상결과를 조사하였다. 수집된 자료는 SPSS/WIN 25.0을 이용하여 유의수준 0.05로 양측 검정을 적용하여 분석하였다. 연구대상자의 일반적 특성은 기술통계로 연속변수 는 평균과 표준편차, 범주형 변수는 빈도와 백분율로 분석하였다. 신속대응팀 활 동 유무에 따른 일반적 특성, 심폐소생술 관련 특성 및 심폐소생술 후 임상결과 및 시간대별 심폐소생술 임상결과의 차이 검정은 Independent T-test,Mann-Whitney U test, Chi-square test 또는 Fisher’s exact test로 차이를 분석하였다. 신속대응팀 활동 유무에 따른 중환자실 생존율은 카플란-마이어 생 존분석(Kaplan-Meier Survival Analysis)으로 파악하였다. 각 그룹별 생존곡선 의 동질성은 로그 순위 검정(log-rank test)을 이용하여 확인하였다.연구결과: 본 연구의 주요 연구 결과는 다음과 같다. 1. 신속대응팀 활동 후 병동 입원환자 1,000명 당 심정지 발생 건수는 1.90건으 로 신속대응팀 활동 전 병동 입원환자 1,000명 당 심정지 발생 건수 2.39건 보다 유의하게 감소하였다 (p
