중환자실은 중증의 환자를 수용하여 24시간 동안 적극적이고 집중적으로 치료하기 위한 병동으로 집중치료실이라고도 한다. 최근 국내에서는 만성질환의 유병율 증가, 노화 현상의 가속화 등의 이유로 인해 중환자실에 입실하는 환자의 수는 증가하고 있으며 이와 함께 의학 기술의 발전으로 중증 질환에 대한 치료 방법 또한 발전하고 있다. 이에 중환자실 간호사의 역량에 대한 관심이 증가하고 있으며 중환자실 간호사의 역량 개발을 위한 연구 또한 다양하게 이루어질 필요가 있다. 그러나 중환자실 간호사는 경력에 따라 역량이 다르게 나타나며 이에 본 연구에서는 중환자실의 신규 간호사와 경력 간호사의 중환자 간호역량에 대한 인식 구조를 탐구하고 비교하여 실무 교육 자료의 기반으로 활용될 수 있도록 하고자 한다.고려대학교 안암병원에서 운영 중인 중환자실인 외과계 중환자실, 내과계 중환자실, 심혈관계 중환자실, 응급 중환자실 총 네 곳의 중환자실에서 신규 간호사 15명, 경력 간호사 15명을 선정하였다. 자료 수집은 2023년 5월 1일부터 2023년 7월 31일까지 진행되었으며 미리 준비된 질문지와 참고자료를 제시 후 인터뷰를 진행하였다. 자료 분석은 Dahlgren & Fallsberg (1991)가 제안한 7단계 분석 방법을 사용하였으며 중환자실의 신규 간호사와 경력 간호사의 중환자 간호역량에 대한 인식구조를 구성하는 범주와 산출 공간을 도출하였다. 연구 결과 신규 간호사와 경력 간호사의 중환자 간호역량에 대한 인식 범주는 간호역량과 간호역량 증진을 위한 전략 두 가지 측면에서 도출되었다. 중환자실 신규 간호사의 간호역량에 대한 인식은 ‘질환과 연계된 발생 가능한 문제의 알고리듬 파악’, ‘처치의 신체적 인과관계 파악’, ‘환자의 상태 변화 과정을 식별’로 총 세 개의 범주가 도출되었으며, 중환자실 신규 간호사의 간호역량 증진을 위한 전략에 대한 인식은 ‘오류 감소를 위한 치료적 처치의 숙지’의 범주에서 ‘개선을 위한 인적 자원관리’, 그리고 ‘중환자실에서 발생하는 다양한 사례별 패턴 확인’으로 총 세 개의 범주가 도출되었다. 또한 중환자실 경력 간호사의 간호역량에 대한 인식은 ‘환자의 취약성에도 불구한 간호 술기의 정확성’의 범주에서 ‘비정상적 징후 식별과 순차적으로 발생하는 증상들과의 연계’, ‘직관을 통해 구성된 알고리듬과 처치의 타당성 확인’, 그리고 ‘중요성 중에서 우선의 중요성 식별’으로 총 네 개의 범주가 도출되었으며 중환자실 경력 간호사의 간호역량 증진을 위한 전략에 대한 인식은 사소한 간호단위를 온전한 간호단위로 인식’, ‘환자의 사소한 문제를 전체적 상황 인식으로 병합’, ‘다직종과의 관계적 관리를 중환자실 자원관리로 연계’로 총 세 개의 범주가 도출되었다. 본 연구결과를 통해 중환자실의 신규 간호사와 경력 간호사의 간호역량에 대한 인식과 간호역량 증진을 위한 전략을 확인할 수 있었으며 이는 경력에 따른 간호사의 역량을 비교하고 경력에 따라 필요한 실무 교육의 기틀을 제시할 수 있도록 하였다.
Background: The intensive care unit is a ward for active and intensive treatment for 24 hours by accommodating severe patients. Recently, the number of patients admitted to intensive care units is increasing due to the increase in the prevalence of chronic diseases and the acceleration of aging, and treatment methods for severe diseases are also developing with advances in medical technology. Accordingly, interest in the competence of intensive care unit nurses is increasing, and various studies to develop the competence of intensive care unit nurses need to be conducted. However, intensive care unit nurses have different competencies depending on their careers, so research on competencies according to careers is needed.Purpose: The purpose of this study is to explore and compare the perception structure of critical care nursing competency of new nurses and experienced nurses in intensive care units so that they can be used as a basis for practical educational data.Data collection and Analysis: For data collection, 15 new graduated registered nurses and 15 experienced nurses were selected from four intensive care units operated by Korea University Anam Hospital. Data collection was conducted from May 1, 2023 to July 31, 2023 by interviews.Findings: As a result of this study, the categories of perception of critical care nursing competency of new and experienced nurses were derived from two aspects: nursing competency and strategies for improving nursing competency. 3 perceptual categories of the nursing competency of new nurses in intensive care units were derived: ‘Identifying algorithms for possible problems linked to diseases’, ‘Identifying the physical causal relationship of treatment" and ‘Identifying the process of changing the patient's condition’, and 3 perceptual categories of strategies for improving nursing competences of new nurses in intensive care units were derived: ‘Acquainted with therapeutic treatment for error reduction’, ‘Human resource management for improvement" and ‘Checking various case-specific patterns occurring in intensive care units’. 4 perceptual categories of the nursing competency of experienced nurses were derived: ‘The accuracy of nursing skills despite the vulnerability of the patient’, ‘Identification of abnormal signs and sequential symptoms’, ‘Verification of the validity of algorithms and treatments constructed through intuition’ and ‘Identification of the importance of priority among importance’. 3 perceptual categories of strategies for improving nursing competences were derived: ‘Recognizing minor nursing units as full nursing units’, ‘Combining minor problems of patients into overall situation awareness’, and ‘Linking relational management with multiple occupations to intensive care unit resource management’.Conclusion: It was confirmed the recognition of nursing competencies and strategies for improving nursing competencies of new nurses and experienced nurses in intensive care units, which allowed them to compare the competencies of nurses according to their careers and present the foundation for practical education necessary according to their careers.
