본 연구는 통계학적 이론에 근거한 엄격한 통제 하에, 5가지 정신건강(정신적・신체적 스트레스, 좌절경험, 기본적 욕구 미충족, 미래에 대한 불안, 과업에 대한 스트레스)에 미치는 금연 및 적정음주의 순수한 효과를 제시하고자 하였다.이를 위해, 한국의료패널 1기 데이터(2011년∼2013년, 2016년∼2018년)를 활용하였으며, 금연의 효과를 파악하기 위해서는 만 19세 이상의 남성, 적절한 음주의 효과를 파악하기 위해서는 만 19세 이상의 성인 모두를 대상으로 하였다. 금연 및 적정음주가 정신건강에 미치는 효과를 파악하기 위해 각각 현재 금연군과 지속적 흡연군, 현재 적정음주군과 지속적 고위험음주군에 대해 성향점수역확률 가중치(Inverse probability of treatment weighting, IPTW)를 활용하여, 각각의 비교 두 집단 간의 동질성을 확보하였다. 이후 금연 전후, 적정음주 전후 각각의 정신건강상태 차이와 동일 시점에서의 정신건강상태 차이를 서로 비교하기 위해 이중차분법(Difference in difference, DID)을 활용하였다. 마지막으로 선행연구에서 제시된 정신건강에 미치는 영향요인을 통제한 후, 통계적 모형으로서 금연 및 적정음주의 순수한 효과를 파악하고자 집단간, 시기간 그리고 이를 모두 고려한 상호작용항을 독립변수로 하는 이중차이를 활용한 다중회귀분석을 실시하였다.주요 연구결과는 다음과 같다.첫째, 흡연에서 금연으로 건강행태에 긍정적인 변화가 있는 경우, 5가지 정신건강상태 변화 중 좌절 경험에서 통계적으로 유의한 차이가 있는 것으로 나타났다.둘째, 금연의 순수한 효과를 파악하기 위해 통제변수를 적용한 결과, 정신적‧신체적 스트레스와 좌절경험 두 가지 정신건강 문항에서 통계적으로 유의한 차이가 있는 것으로 나타났다.셋째, 고위험음주에서 적정음주로 건강행태에 긍정적인 변화가 있는 경우, 정신적‧신체적 스트레스, 기본적 욕구 미충족, 미래에 대한 불안 문항에서 통계적으로 유의한 차이가 있는 것으로 나타났다.넷째, 적정음주의 순수한 효과를 파악하기 위해 통제변수를 적용한 결과, 미래에 대한 불안 문항에서 통계적으로 유의한 차이가 있는 것으로 나타났다.본 연구결과, 금연은 정신적‧신체적 스트레스와 좌절 경험을 감소시키고, 적정음주는 미래에 대한 불안을 감소시키는 중요한 요인인 것으로 나타났다. 이로써 금연 및 적정음주와 같은 건강행태의 긍정적인 변화와 정신건강간 서로 밀접한 관련이 있는 것으로 나타났으며, 시기변수의 적용을 통해 건강행태와 정신건강 사이의 인과관계를 명확하게 파악할 수 있었다.이러한 결과를 고려할 때, 정신건강 문제가 있는 인구의 흡연 및 음주습관과 니코틴 및 알코올 의존도에 대해 주의 깊은 관심을 보여야 할 것이며 이러한 연구를 바탕으로 특화된 보건교육프로그램 개발이 필요할 것으로 보여진다.
This study aimed to present the pure effects of smoking cessation and moderate alcohol consumption on five aspects of mental health (mental and physical stress, experiences of frustration, unmet basic needs, anxiety about the future, and stress related to tasks) under strict control based on statistical theory. For this purpose, data from the first Korean Medical Panel (2011-2013, 2016-2018) was utilized, targeting men over 19 for the effects of smoking cessation, and all adults over 19 for the effects of moderate drinking. To ascertain the effects of smoking cessation and moderate drinking on mental health, inverse probability of treatment weighting was utilized to ensure homogeneity between the current non-smoking group and the continuous smoking group, and the current moderate drinking group and the continuous high-risk drinking group. The difference in difference method was then used to compare the differences in mental health status before and after smoking cessation and moderate drinking, and at the same time. Finally, after controlling for factors affecting mental health as presented in previous research, multiple regression analysis using the difference-in-difference approach was conducted to determine the pure effects of smoking cessation and moderate drinking. This analysis included interaction terms considering the differences between groups and over time, as independent variables, within the statistical model. The main results of the study are as follows: First, a statistically significant difference was found in experiences of frustration among the five mental health status changes when there was a positive change in health behavior from smoking to non-smoking. Second, after applying control variables to ascertain the pure effects of smoking cessation, statistically significant differences were found in two mental health items: mental and physical stress and experiences of frustration. Third, in the case of a positive change in health behavior from high-risk drinking to moderate drinking, statistically significant differences were found in mental and physical stress, unmet basic needs, and anxiety about the future. Fourth, after applying control variables to determine the pure effects of moderate drinking, a statistically significant difference was found in the item related to anxiety about the future. The results of this study indicate that smoking cessation reduces mental and physical stress and experiences of frustration, while moderate drinking decreases anxiety about the future. This demonstrates a close relationship between positive changes in health behaviors such as smoking cessation and moderate drinking, and mental health. The application of time variables allowed for a clear understanding of the causal relationship between health behaviors and mental health. Given these outcomes, it's crucial to closely monitor the smoking and alcohol consumption behaviors, alongside nicotine and alcohol dependencies, in individuals with mental health conditions. Based on this study, there appears to be a need for the development of specialized health education programs.
