This study compares general characteristics and quality of life-related health behaviors (drinking, smoking, vaccination, and physical activity) between solitary older individuals aged 65–74 years and those aged ≥75 years. It aimed to contribute fundamental research data through secondary analysis for improving and managing older individuals’ health and quality of life. Among 7,090 respondents to the 8th Korea National Health Nutrition Examination Survey conducted by the Korea Disease Control and Prevention Agency, 462 participants who satisfied the inclusion criteria were included in the analysis. The IBM SPSS Statistic version 26 was used for data analysis. Chi-squared test, independent t-test, and ANOVA were conducted to examine and compare health behaviors and the quality of life between the age groups.Health-related quality of life, the dependent variable, was assessed using the Health-related Quality of Life Instrument with 8 Items (HINT-8), specifically developed to measure health-related quality of life among Koreans. Health behaviors were considered as independent variables, which included drinking (monthly drinking frequency and amount of alcohol consumed per occasion), smoking (current smoking status), flu vaccination (vaccination status), physical activity (amount of time spent sitting down, number of days walking in a week, engagement in muscle-strengthening exercise in a week, and engagement in aerobic exercise in a week), and general characteristics (sex, education level, economic status, basic livelihood status, income level, health check-ups, subjective health perception, diagnosis of hypertension, hyperlipidemia, diabetes, and/or arthritis, body mass index, and number of comorbidities).In the analysis, not receiving health check-ups, being diagnosed with hypertension and/or arthritis, and not being diagnosed with hyperlipidemia were more significantly associated with being female, completing education up to middle school or less, and not engaging in economic activity in older individuals aged ≥75 years, than in those aged 64–75 years. The older individuals aged ≥75 years had significantly less engagement with aerobic exercise if their monthly alcohol consumption was low, drank 1–2 cups of alcohol in one sitting, did not smoke at present, spent more hours sitting down per day, and spent fewer days walking in a week.Regarding the general characteristics and health-related quality of life, completing high school education or above, not receiving basic livelihood support, having higher income and subjective health perception, not being diagnosed with arthritis, and having 0–1 comorbidities were significantly associated with a higher quality of life in older individuals aged 65–74 years. For the older individuals aged ≥75 years, not receiving basic livelihood support, having higher subjective health perception, not being diagnosed with hyperlipidemia, diabetes and/or arthritis, and having 0–1 comorbidities were significantly associated with a higher quality of life. A higher health-related quality of life was significantly associated with less time spent sitting down per day in the older individuals aged 65–74 years and with not being vaccinated in those aged ≥75 years. The results of this study suggest the need for research on improving physical activity levels tailored to age and sex characteristics, as well as comorbidity prevention and management in the older population. Although the conclusions of this study are limited, they may provide useful foundational data for improving the quality of life in older individuals.
본 연구는 만 65세 이상 독거노인을 대상으로 하여 만 65-74세 노인과 75세 이상 노인으로 분류하고 일반적 특성과 건강행위(음주, 흡연, 예방접종, 신체활동)에 따른 건강관련 삶의 질을 분석하여 노인의 건강증진 및 관리와 삶의 질 개선을 위한 기초자료를 제시하고자 시도된 2차 자료 분석 연구이다. 이를 위해 2021년에 질병관리본부에서 시행한 국민건강영양조사 제 8기 원시자료를 이용하여 총 응답자 7,090명 중에서 만 65세 이상이며 연구 대상자 기준에 해당하지 않는 자를 제외하여 최종 연구 대상자 462명의 자료를 분석하였다. 자료 분석은 IBM SPSS Statistic ver.26 프로그램을 이용하였다. 65-74세 노인과 75세 이상 노인의 건강행위와 삶의 질의 특성을 알아보고 비교하기 위해 카이제곱 분석, independent t-test, ANOVA를 시행하여 분석하였다.본 연구의 종속변수는 건강관련 삶의 질이고 측정도구로는 한국형 건강관련 삶의 질 측정도구인 HINT-8(HRQOL instrument with 8 items)이 사용되었다. 독립변수는 건강행위로 음주(월간 음주율, 한번에 마시는 음주량), 흡연(현재 흡연여부), 독감 예방접종(독감 예방접종 여부), 신체활동(평소 하루 앉아서 보내는 시간, 1주일간 걷기 일수, 1주일간 근력운동 실천 여부, 유산소 신체활동 실천 여부), 일반적 특성(성별, 교육수준, 경제활동 상태, 기초생활 수급 여부, 소득수준, 건강검진 수진 여부, 주관적 건강인지, 고혈압 의사진단 여부, 이상지질혈증 의사진단 여부, 당뇨병 의사진단 여부, 관절염 의사진단 여부, 체질량지수, 동반 질환수)이 사용되었다.본 연구를 분석한 결과 일반적 특성으로는 65~74세 노인보다 75세 노인에서 성별 ‘여성’, 교육수준 ‘중졸이하’, 경제활동 ‘하지 않음’, 소득수준이 낮을수록, 건강검진 수진 ‘받지 않음’, 고혈압 의사진단 ‘있음’, 이상지질혈증 의사진단 ‘없음’, 관절염 의사진단 ‘있음’의 특성이 유의하게 나타났다. 건강행위로는 65~74세 노인보다 75세 노인에서 월간 음주율 ‘낮음’, 한번에 마시는 음주량 ‘1-2잔’, 현재 흡연여부 ‘하지 않음’, 평소 하루에 앉아서 보내는 시간이 많을수록, 1주일간 걷기 일수가 적을수록, 유산소 신체활동 실천여부 ‘하지 않음’이 유의하게 나타났다. 일반적 특성과 건강관련 삶의 질에서는 65~74세 노인은 교육수준 ‘고졸이상’, 기초생활수급 경험 ‘없음’, 소득수준이 높을수록, 주관적 건강인지가 좋을수록, 관절염 의사진단 ‘없음’, 동반 질환수 ‘0-1개’에서 건강관련 삶의 질이 높은 것으로 유의하게 나타났다. 75세 이상 노인은 기초생활수급 경험 ‘없음’, 주관적 건강인지가 좋을수록, 이상지질혈증 의사진단 ‘없음’, 당뇨병 의사진단 ‘없음’, 관절염 의사진단 ‘없음’, 동반 질환수 ‘0-1개’에서 건강관련 삶의 질이 높은 것으로 유의하게 나타났다. 건강행위에 따른 건강관련 삶의 질에서는 65~74세 노인은 평소 하루 않아서 보내는 시간이 적을수록, 75세 이상 노인은 예방접종을 받지 않은 경우가 건강관련 삶의 질이 높은 것으로 유의하게 나타났다.이를 바탕으로 독거노인의 연령과 성별의 특성을 고려한 신체활동량을 증가시킬 수 있는 연구와 동반 질환의 예방 및 관리가 필요하다. 본 연구에서의 결론이 작으나마 노인의 삶의 질을 높이는 기초자료로 활용되길 기대한다.
