우울증은 경도인지장애(MCI)에서 치매로 진행되는 위험인자로 간주 되기 때문에 경두개 직류자극(tDCS) 중재를 받는 MCI 환자들의 우울증 개선이 임상적 우선순위로 고려되어야 한다. 본 연구는 인지적, 심리적, 전기생리학적 지표를 통합하여 MCI 및 우울증 환자 모두에게 가정용 원격 모니터링 tDCS의 효능을 종합적으로 평가하는 것을 목표로 했다. 6주간의 무작위, 이중 맹검 및 대조군 tDCS 연구로 37명의 지역사회 거주 대상자를 실험 자극 집단 또는 대조 자극 집단에 무작위로 할당했다. 양극(Anode)은 왼쪽에, 음극(Cathode)은 오른쪽 배측 전전두피질에 위치한 상태에서 30분 동안 2mA의 가정용 tDCS를 30일간 적용 받았다. 본 연구에서는 우울증, 신경 인지 기능, 그리고 뇌파(EEG)를 평가 도구로 측정했다. 결과적으로, tDCS의 적용을 통한 우울 증상과 인지 기능의 개선에 대한 효과적 측면에서 실험 집단과 대조 집단 간에 유의미한 차이는 없었다. 그러나 대조 집단에 비해 실험 실험 집단의 정량화 뇌파 결과에서 델타파의 감소와 베타파의 증가가 나타났다. 또한 베타파의 증가는 실험 집단에서만 인지 기능 향상과 상관관계가 나타냈다. MCI 환자와 우울증 환자 모두에게서 우울증과 인지에 대한 tDCS의 효능에 대해서는 명확한 결론을 도출할 수 없었다. 그럼에도 tDCS에 의한 전기생리학적 변화와 인지 기능 사이의 관계는 인지과정에 관련된 잠재적인 신경가소성의 향상을 시사한다.
Since depression is considered a risk factor for progression from mild cognitive impairment (MCI) to dementia, improving depression in MCI patients receiving transcranial direct current stimulation (tDCS) intervention should be considered as a clinical priority. This study aimed to comprehensively evaluate the efficacy of home remote monitoring tDCS for both patients with MCI and depression by integrating cognitive, psychological and electrophysiological indicators. A 6-week, randomized, double-blind, and controlled tDCS study randomly assigned 37 community-dwelling subjects to either the experimental stimulus group or the control stimulus group. With the anode located on the left side and the cathode located on the right dorsolateral prefrontal cortex, 2mA home-use tDCS was applied for 30 minutes for 30 days. In this study, depression, neurocognitive function, and electroencephalogram (EEG) were measured as assessment tools. As a result, there was no significant difference between the experimental group and the control group in terms of the effectiveness of improving depressive symptoms and cognitive function through the application of tDCS. However, compared to the control group, the quantified EEG results of the experimental group showed a decrease in delta waves and an increase in beta waves. In addition, the increase in beta waves showed a correlation with cognitive function improvement only in the experimental group. No clear conclusions could be drawn about the efficacy of tDCS on depression and cognition in both patients with MCI and depression. Nonetheless, the relationship between tDCS-induced electrophysiological changes and cognitive function suggests an enhancement of potential neuroplasticity related to cognitive processes.
