목적: 하위 그룹을 명확하게 언급하는 달리 분류되지 않은 정신병적 장애(Psychotic disorder not otherwise specified, PNOS)에 대한 연구는 매우 드물다. 이 연구는 초기 단계의 조현병(schizophrenia, SZ) 환자와 달리 분류되지 않은 정신병적 장애 환자사이에 인구통계학적 특징과 임상적 특징, 인지 기능, 1년 추적 임상결과를 확인하기 위해 수행되었다.방법: 연구는 등록된 지 1년이 경과한 달리 분류되지 않은 정신병적 장애 환자 54명과 조현병 환자 321명을 대상으로 조사하였다. 1년 추적기간 동안 탈락된 37명의 달리 분류되지 않은 정신병적 장애 환자 및 210명의 조현병 환자는 1년 시점에서 평가하였다. 두 그룹 간의 임상 변수(치료되지 않은 정신병 기간, 증상 심각도, 자가 평가 척도 등), 인지 기능, 단기간의 결과(치료 반응, 관해, 순응도, 탈락, 재발)를 비교하였다. 결과: 달리 분류되지 않은 정신병적 장애 환자는 이전 연구에 비해 1년 진단 안정성이 53.7%로 더 높았다. 그들은 조현병 환자에 비해 증상 심각도가 낮았고, 2개월째 치료 반응이 더 좋았고, 12개월째 관해율이 더 높았지만, 6개월째 순응도는 오히려 낮았다. 달리 분류되지 않은 정신병적 장애 환자의 인지 장애 수준은 조현병 환자와 건강한 대조군의 중간 수준이었다.결론: 이러한 결과는 달리 분류되지 않은 정신병적 장애 환자가 독특한 임상적 소견을 가지고 있음을 시사하며, 이는 별개의 임상 증후군으로 분류되어야 할것으로 생각된다. 그러나 일부 달리 분류되지 않은 정신병적 장애 환자에서 다른 정신병적 장애로 진행하는 것을 예방하는 것도 중요하다고 생각한다.
Objectives: Research on psychotic disorder not otherwise specified (PNOS) that clearly mentions its subgroups is very rare. This study was conducted to identify the demographic and clinical features, cognitive function, and 1-year outcomes of patients with early-stage PNOS compared to those with early-stage schizophrenia (SZ). Methods: The number of the subjects were 54 and 321 patients with PNOS and SZ, respectively, who were registered at least more than 1 year ago. Due to drop out, only 37 and 210 patients with PNOS and SZ were evaluated at the 1-year follow-up. Author compared clinical variables (duration of untreated psychosis [DUP], symptom severity, self-rating scales, etc.), cognitive function, and short-term outcomes (treatment response, remission, compliance, drop out, relapse) between the two groups at baseline, 2-, 6-, and 12-months.Results: The patients with PNOS were associated with higher diagnostic stability (53.7%) compared to those in previous studies. They had lower symptom severity, better treatment response at 2 months and higher remission rates at 12 months, but poorer drug compliance at 6 months compared to patients with SZ. Level of cognitive impairment in PNOS was intermediate between those of SZ patients and healthy controls.Conclusions: These findings indicate that PNOS has unique clinical features, suggesting that it should be treated as a distinct clinical syndrome. At the same time, however, prevention of its possible progression to other psychotic disorders in some patients with PNOS is also important.
