지금 과학의 발달과 경제 수준의 향상에 따른 생활양식의 변화와 함께 의학이 발달하면서 인간의 수명이 연장되고 만성질환과 말기환자의수가 증가하게 되었다. 경제 수준의 향상 때문에 삶의 질에 관심을 가지게 되어 말기환자들의 고통을 제거하고 편안한 삶을 유지하고자 하는 요구가 팽배되는 시점에 이르렀다. 이러한 말기환자를 돌보기 위해 발달한 것이 호스피스라 하겠다. 국가의 의료와 복지의 목표가 국민의 삶의 질 향상이며 복지사회가 추구하는 것이 궁극적으로 인간의 존엄성이다. 따라서 말기환자의 경우에도 인간다운 생활을 유지케 하기 위해 사회에서 이용 가능한 인적, 물적, 자원을 최대한 활용하여 임종을 앞둔 말기환자들의 마지막 생을 보다 편안하게 보낼 수 있도록 하는 것이 인간적인 삶에 대한 배려이다. 본 연구는 호스피스의 개념을 분석하고 호스피스 도입을 위한 제반여건을 조정하여 중국 보건의료체계의 한 부분으로 정착될 수 있는 호스피스 제도의 문제점과 개선방안을 제시하는데 그 목적이 있다. 중국 호스피스 발전을 위해서 무서보다도 먼저 호스피스의 현황과 문제점을 파악괴어야 한다. 이에 본 연구에서는 문헌조사를 통하여 중국 호스피스 현황에서 문제점을 도출하고 다른 나라의 호스피스현황을 참고하며 중국의 상황에 적합한 개선방안을 제시하였다. 본 연구는 길버트와 스펙트의 분석들을 적용하여 중국 호스피스 제도를 분석하였다. 이 본석틀은 대상, 급여, 전달체계, 재원으로 나눠서 중국 호스피스의 문제점을 잘 알게 될 수 있다. 호스피스가 효율적인 운영으로 적합한 중국형 호스피스 제도를 만들어야 한다. 그리고 의사, 간호사, 사회복지사 등 각 분야의 전문 인력을 양성해야 한다. 마지막으로 국민들이 호스피스에 대한 인식을 바꾸고 호스피스 의 이용률을 높아지면 좋겠다. 주요어: 말기 암 환자, 호스피스, 중국 호스피스의 문제점, 길버트와 스펙트의 분석틀
The progress for science and technology has brought about rapid changes in people's of way of lives. It has also prolonged people's span of lifes, and increased the number of chronological disease and the number of patients at their last moment.The higher level of economic well-being has caused people to be concerned about the quality of lives of such patients at their last moment. The hospice system has been developed in order to take good care of them.The ultimate purpose of medical service and welfare is said to be the improvement of people's quality of lives and the respect of human dignity.Accordingly the patients at their last moment also have the right to be taken good care of. We should have much more consideration in order to make them spend their last lives more comfortably. This paper aims to explain the concept of hospice system, to describe the present situation of hospice and the medical care for the patients at their last moment in China, to compare the results with those of advanced countries, and to derive some policy implication and policy measures to cope with such problems. We largely depend upon literature survey for carrying out this research. Most of them are religiously related organization, and are not formally institutionalized as a public health care system. Therefore, no standards have yet been established for the hospice activities and their and their organization. Furthermore, there has not special education program as well as information of the hospice system. Accordingly, there has been very few specialist on hospice and average citizens seldom have the knowledge of hospice. This paper aims to explain the concept of hospice system, to describe the present situation of hospice and the medical care for the patients at their last moment in China. In this paper, I suggest that legal system as well as policy support should provided in order to activate hospice system. We also suggest that a special program should be developed for the education of doctor, nurses, social workers who can work at hospice. Finally, I'm suggest that a very positive public relations programs should be worked out in order to derive wide public support for the early introduction and institutionalization of hospice system.
