2000년부터 중국은 노령화 사회에 진입하면서 노년기 건강에 대한 사회적 관심이 고조되고 있다. 더불어 사회경제적으로 급속히 발전되는 중국 사회에서 노인들의 건강 형태 및 특성, 그리고 욕구도 다양해지고 있다. 한편 개인의 건강은 전 생애과정을 걸쳐 유동적이고 역동적으로 변화될 뿐만 아니라 건강의 변화 패턴은 노인들의 특성에 따라 다층적으로 전개될 수 있다. 이처럼 노인 집단 내의 건강 이질성과 건강 변화궤적의 불균형성에 주목하여, 본 연구는 중국 노인의 신체 및 정신건강 결합 변화궤적을 탐색하고, 신체 및 정신건강 결합 변화궤적에 따른 잠재집단을 유형화하고, 각 집단별로 특성을 파악고자 하였다. 아울러, 중국 노인의 신체 및 정신건강 결합 변화궤적에 따른 다양한 유형을 결정하는 요인들을 규명하고자 하였다. 본 연구에 활용한 데이터는 중국 전역의 노인을 대상으로 조사된 "Chinese Longitudinal Healthy Longevity Survey" 패널 자료이며, 분석에는 2002년부터 2011년까지 총 10년간의 원(原) 자료를 사용하였다. 분석대상을 65-75세 노인으로 설정하였다. 한편 노인의 신체 및 정신건강 결합 변화궤적을 분석하기 위하여 건강변수는 신체 및 정신적 건강을 함께 측정하였다. 노인 건강의 변화궤적에 따른 유형에의 결정요인으로 생물유전학적 요인, 사회경제적 요인, 사회적 지지 및 관계 요인, 문화행동적 요인, 물리환경적 요인을 채택하였다. 분석방법으로는 연구 대상의 건강 및 일반적 특성을 파악하기 위하여 빈도분석, 기술통계분석을 사용하였다. 중국 노인의 신체 및 정신건강 결합 변화궤적의 유형화분석에는 성장혼합모형을 활용하였다. 또한 중국 노인의 신체 및 정신건강 변화궤적의 유형 집단에 속한 노인의 특성을 차이를 확인하기 위하여 χ2 test와 ANOVA 분석을 하였다. 마지막으로 각각의 잠재집단에 결정하는 요인들을 규명하기 위하여 이항 및 다항 로지스틱 회귀분석을 실시하였다. 본 연구에서 확인된 주요 분석결과들은 다음과 같다. 첫째, 전체 분석대상을 하나의 동질적 집단으로 가정할 경우, 시간이 지남에 따라 노인의 건강수준이 전반적으로 감소하는 양상이 나타났다. 그러나 중국 노인 신체정신 건강궤적 각각의 추정치를 구체적으로 살펴보면, 신체건강(만성질환의 수)과 정신건강(취약수준) 수준의 모수변량 초기값과 변화율에서 모두 유의미한 차이가 있는 것으로 분석되었다. 이는 중국 노인들은 노화에 따른 신체 및 정신건강의 변화에서 상당한 이질성을 존재한다는 의미이며, 중국 노인의 건강 변화궤적은 단일궤적이 아닌 다양한 변화궤적 유형을 존재함이 확인되었다. 둘째, 본 연구는 성장혼합모형을 통하여 중국 노인의 건강 변화궤적에 대하여 유형화한 결과, 중국 노인의 신체 및 정신건강 결합 변화궤적에는 ‘신체질환정신취약 증가형’, ‘신체질환 감소정신취약 중수준 유지형’, ‘신체질환 저수준정신취약 고수준 유지형’, ‘신체질환정신취약 저수준 유지형’의 네 가지 상이한 잠재집단이 존재하는 것으로 분석되었다. 아울러 중국 노인의 신체 및 정신건강의 결합 변화 양상이 서로 동일한 방향으로 변화하기보다 상당히 복합하고 다양한 변화 패턴으로 전개된다는 것이 확인되었다. 셋째, 신체 및 정신건강 결합 변화궤적에 따른 각 잠재집단의 결정요인을 분석한 결과, 성별, 연령, 교육 연수, 가구 소득, 의료보험 가입 여부, 경제적 지지와 도구적 지지 여부, 흡연량과 음주량, 거주 지역 변수는 신체질환정신취약 증가형을 결정하는 것으로 나타났다. 그 다음으로 신체질환 감소정신취약 중수준 유지형을 판별하는데, 연령, 교육연수, 의료보험 가입 여부, 경제적 지지와 도구적 지지 여부, 사회활동 참여는 유의미한 영향을 미치는 것으로 확인되었다. 또한 성별, 교육수준, 소득 수준, 의료보험 가입 여부, 독거 여부, 정서적 지지 여부, 사회활동 참여 수준, 운동 여부, 거주 지역 변수가 신체질환 저수준정신취약 고수준 유지형을 결정한 것으로 분석되었다. 마지막으로 신체질환정신취약 저수준 유지형을 결정하는 요인으로서 성별, 연령, 교육 수준, 소득 수준, 의료보험 가입 여부, 도구적 지지 유무, 사회활동 참여 수준, 운동 여부인 것으로 나타났다. 본 연구가 지닌 주요한 함의는 다음과 같다. 첫째, 본 연구는 단일 건강문제 특정 모델을 적용한 기존 건강연구와 달리, 신체건강과 정신건강변화의 불균형성에 주목하여 중국 노인의 신체 및 정신건강 결합 변화궤적의 잠재집단을 분석하였다. 분석결과에서 중국 노인의 신체 및 정신 건강변화는 일정한 방향으로 흘러가는 양상이 존재하기도 하지만, 신체와 정신 건강변화가 일관되지 않고, 다양한 변화 형태로 나타나는 것으로 확인되었다. 이처럼 본 연구는 건강궤적 다양성뿐만 아니라 각 건강지표 간의 궤적변화 불균형성을 보여줌으로써, 다변(多變)하는 노인 건강의 특성을 제대로 파악하지 못하는 선행연구들의 한계를 보완하는데 일련의 의의가 지닌다. 또한 중국 노인들의 건강상태를 정교하게 파악하기 위하여 종합적인 건강검진과 정보 시스템 구축이 필요함을 시사해준다. 둘째, 본 연구에서 중국 노인 신체정신 건강변화에는 매우 다양한 패턴이 존재하는 것이 확인되었다. 따라서 중국 노인 건강수준을 관리하고 향상시키기 위하여 노인의 건강상태와 건강발달양상별로 관련 프로그램들이 다르게 제공되어야 한다. 다시 말하면, 중국 노인의 통합적인 건강수준별 집단의 특징에 따라 구분하고, 각 수준에 맞는 개입이 이루어지도록 해야 할 것이다. 셋째, 본 연구는 중국 노인의 신체건강과 정신건강의 변화 시점이 다를 수 있음을 확인하였다. 곧 신제 질환을 증가하는 직후에 바로 정신건강 악화가 진행되지 않을 수 있다는 것이다. 따라서 건강궤적 유형 특성에 따른 차별화 개입할 동시에, 건강의 변화 시기에 맞추어 적절게 개입하는 것도 매우 중요한다. 노인의 건강관리를 효과적으로 하려면 건강에 대한 일시적인 검사가 아닌 장기적인 관심이 기울려야 할 것이다. 특히 신체정신적 건강에서 모두 취약한 노인 집단들의 건강관리에 있어서, 더욱 도움이 될 수 있을 것이다.
Social interest has increased in health in old age as China has entered into an aging society since 2000. In addition, in Chinese society with rapid social and economic developments, the types, characteristics, and needs for the health of the elderly become more diverse. Meanwhile, personal health flexibly and dynamically changes over the entire life process and the pattern of change in health may develop in multiple layers according to the characteristics of the elderly. This study would explore conjoint trajectories of physical and mental health among the Chinese elderly, classify latent classes according to conjoint trajectories of physical and mental health and understand the characteristics of each latent class, paying attention to the heterogeneity of health among elderly and the imbalance of trajectories of health. In addition, this study would investigate the factors determining the latent classes according to the conjoint trajectories of physical and mental health among the Chinese elderly. The data utilized in this study are the panel data of "Chinese Longitudinal Healthy Longevity Survey" conducted with the elderly throughout China. For analysis, source materials from 2002 through 2011 were used. Subjects of analysis were the elderly aged between 65 and 75. Meanwhile, to analyze conjoint trajectories of physical and mental health among the Chinese elderly, for health variables, their physical health and mental health were measured together. As the determinants of the types according to the trajectories of the health among the Chinese elderly, biological/genetic factors, social/financial factors, social support and relationship factors, cultural/behavioral factors, physical/environmental factors were adopted. As for the method of analysis, to understand the health and general characteristics of the research subjects, frequency analysis, and descriptive statistical analysis were conducted. For an analysis of the classification of conjoint trajectories of physical and mental health among the Chinese elderly, a growth mixture model was utilized. In addition, to check the differences in the characteristics of the elderly that belong to the classes of the types of the trajectories of physical and mental health among the Chinese elderly, χ2 test and ANOVA analysis were conducted. Lastly, to investigate the determinants of each latent class, binomial and multinomial logistic regression analysis were conducted. The main results of the analysis found out in this study are as follows: First, if the entire subjects of analysis are assumed to be one homogeneous class, the health level of the elderly tended to decrease overall as time passed by. However, to examine the estimation of each of the trajectories of physical and mental health among the Chinese elderly, concretely, it turned out that there were significant differences in both the initial value of parameter variance and the rate of change in the level of physical health (The number of chronic diseases) and the level of mental health (Vulnerability level). This means that there is considerable heterogeneity in changes in the physical and mental health among the Chinese elderly according to aging, and it was noted that in the trajectories of health among the Chinese elderly, there were various types of trajectories instead of a single trajectory. Second, this study classified the trajectories of health among the Chinese elderly through a mixed growth model, and as a result, there were four different latent classes including “the type of increases in physical diseases/mental vulnerabilities”, “the type of decreases in physical diseases and the maintenance of the medium level of mental vulnerabilities”, “the type of the maintenance of the low level of physical diseases and the maintenance of the high level of mental vulnerabilities” and “the type of the maintenance of the low level of physical diseases and mental vulnerabilities” in the conjoint trajectories of physical and mental health among the Chinese elderly. Along with this, it was found out that the aspects of conjoint changes in the physical and mental health among the Chinese elderly develop in quite complex various change patterns rather than changing in the same direction. Third, as a result of an analysis of the determinants of each latent class according to the conjoint trajectories of physical and mental health, the variables such as sex, age, education and training, household income, medical insurance subscription, economic support and instrumental support, the amount of smoking and the amount of drinking, and residential area determined the type of increases in physical diseases and mental vulnerabilities. Next, it turned out that in differentiating the type of decreases in physical diseases and the maintenance of the medium level of mental vulnerabilities, age, education and training, medical insurance subscription, economic support and instrumental support, and participation in social activities had significant impacts. In addition, it turned out that the variables, including sex, education level, income level, medical insurance subscription, whether to live alone, emotional support, the level of participation in social activities, exercise, and residential area determined the type of the maintenance of the low level of physical diseases and the maintenance of the high level of mental vulnerabilities. Lastly, it turned out that the factors determining the type of the maintenance of the low levels of physical diseases and mental vulnerabilities included sex, age, education level, income level, medical insurance subscription, instrumental support, the level of participation in social activities, and exercise. The main implications of this study are as follows: First, unlike the existing health research applying a single health problem-specific model, this study analyzed the latent classes of conjoint trajectories of the physical and mental health among the Chinese elderly, paying attention to the imbalance of changes in physical health and mental health. The latent classes of conjoint trajectories of physical and mental health among the Chinese elderly were analyzed. In the results of the analysis, changes in the physical and mental health among the Chinese elderly tended to flow in the constant direction, but it was found out that changes in physical and mental health were not consistent and appeared as various types of changes. This study has a set of significance to compensate the limitation of previous studies, which could not understand the characteristics of the changing health of the elderly, properly by showing the imbalance of changes in trajectories between health indices as well as the diversity of trajectories of health. In addition, to understand the status of health of the Chinese elderly elaborately, it is necessary to check health comprehensively and construct the information system. Second, this study found out that there were very various patterns in changes in the physical and mental health of the Chinese elderly. Thus, to manage and enhance the health level of the Chinese elderly, the related programs should be provided differently for the health status of the elderly and the pattern of the development of their health. In other words, it would be necessary to divide the Chinese elderly according to the characteristics of the classes by the integrated health level and to intervene in them according to the level. Third, this study confirmed that the point in time of changes might differ in physical and mental health among the Chinese elderly. In other words, the aggravation of mental health may not progress immediately after the increase of physical diseases. Thus, it is very important to intervene appropriately for the time of changes in health at the same time with the intervention according to the type and characteristic of health trajectories. For the effective health care of the elderly, long-term attention should be paid, instead of the temporary inspection of health. Especially, it would be more helpful for the health care of the classes of the elderly that are vulnerable in both physical health and mental health.
