Generally, weight bearing long bones fractures are fixed with internal fixation devices made of metals and alloys. Stress shielding and refractures are known the most common and worst problems due to higher difference in Young’s modulus of bones and metallic implants. To overcome these problems, fiber reinforced polymer composites are very famous and getting attention of researchers. However, the removal of these composite and metallic devices after complete healing leads to the patient to the pain, blood loss, time for rest, operative cost and so on. Therefore, completely biodegradable composites were developed, which are resorbed when exposed to human body fluid, release the bioactive ions. They transfer the body weight to the fracture site as the material properties are degraded. Appropriate initial mechanical properties and degradation rate of material is required to transfer the appropriate body weight at appropriate time for the successful healing of bone fractures. In our study, 3D model of human tibia was generated in ABAQUS 6.10. In our analysis, 10% body weight was used for first 8 weeks, 200% body weight was used for 8-12 weeks (during abnormal walk) and 300% body weight was used for 12-16 weeks of healing simulation of bone fractures. A mechano-regulation theory with a deviatoric strain is the most efficient to predict the bone healing which was programmed in user’s subroutine Python. The most influential design parameters (Young’s modulus, degradation rate of material, functionally graded material and thickness of bone plate) were included in one study to investigate the optimal design of bone plate. In the last, in order to fabricate the real bone plate, basic material properties were evaluated of phosphate glass fiber (PGF) reinforced polylactic acid (PLA) composites. To improve the mechanical properties of these composites, plasma treatment was done on the surface of PGF and the optimal treatment time was determined for PGFs. To check the effect of plasma treatment, bending tests, interlaminar shear tests and tensile tests were performed. To design the real bone plate, the determination of fatigue life is very important because the bone plate is subjected to the cyclic loading. Therefore, loads were applied on the composite specimen according to the FEA results and fatigue life was calculated. Also the degradation rate of material and bioactivity in simulated body fluid PBS were determined at accelerated temperature 50°C. The results showed the plasma treatment affectively improved the mechanical properties of PGF/PLA composites.
일반적으로 체중을 지지하는 대퇴골(femur) 및 경골(tibia)의 골절부는 금속 및 합금으로 제작된 내부 고정판(Bone plate)으로 고정된다. 금속으로 제작된 고정판을 사용하여 골절부를 고정하는 경우 뼈와 고정판 재료 간 상당한 탄성계수의 차이로 인해 응력방패현상(Stress shielding) 및 재골절(refracture)과 같은 심각한 문제가 발생할 수 있다. 이러한 문제를 해결하기 위해 인체의 뼈와 유사한 재료 물성을 가지는 고정판을 제작할 수 있도록 적층각도에 따라 물성의 변화가 자유로운 섬유강화 복합재료(fiber reinforced polymer composites) 고정판에 대한 연구가 주목을 받아왔다. 하지만 섬유강화 복합재료 고정판도 기존 금속 고정판과 마찬가지로 골절 치료가 완료된 후 재수술을 통해 체내에서 고정판을 제거해야 하므로 추가 수술이 필요하며, 이는 환자의 통증 유발, 금전적 손실, 수술 후 휴식 기간 필요 등과 같은 단점이 있다. 따라서 체액에 노출된 상태에서 재흡수 가능한 생분해성 복합재료 (Biodegradable composite material) 고정판에 대한 연구가 진행되었다. 골절부에 적용된 생분해성 복합재료 고정판은 시간이 지남에 따라 체내에서 서서히 분해되며, 이로 인해 골절부에 시간에 따라 변하는 하중을 전달한다. 골절부가 성공적으로 완치되기 위해서는 적합한 하중조건을 적시에 골절부에 전달할 필요가 있으며, 이를 위해 생분해성 복합재료 고정판의 적절한 초기 기계적 물성 선정과 재료 분해 속도 선정이 필요하다. 본 연구에서는 ABAQUS 6.10을 사용하여 사람의 경골을 모델링 하였다. 처음 8주 동안에는 체중의 10% 하중 조건을 부여하였으며, 8주-12주 기간 동안에는 체중의 200%, 그리고 12-16주 동안에는 체중의 300% 하중 조건을 부여하였다. 편향변형률(deviatoric strain)이 기준이 되는 메카노규제이론(mechano-regulation theory)은 골절 치료 효율을 예측하는데 있어 가장 효과적인 방법 중 하나이다. 본 연구에서는 메카노규제이론을 해석적으로 적용함으로써 정확한 치료효율을 계산할 수 있도록 파이썬(Python) 코드를 활용한 유저서브루틴(user’s subroutine)을 프로그래밍 하였다. 골절 치료 효율에 가장 큰 영향을 미치는 요인으로는 고정판의 탄성계수, 재료 분해 속도, 경사기능재료, 고정판의 두께가 있으며, 고정판의 최적 설계를 진행하는 과정에서 해당 요인들을 고려하였다. 마지막으로, 실제 고정판을 제작하기 위해 phosphate glass fiber (PGF) reinforced polylactic acid (PLA) 복합재료에 대한 기본적인 물성 평가를 수행하였다. 생분해성 복합재료의 기계적 물성을 향상시키기 위해 PGF의 표면에 플라즈마 처리를 진행하였으며, 간단한 실험을 통해 최적의 플라즈마 처리 시간을 결정하였다. 플라즈마 처리의 효과를 검증하기 위해 휨 실험(bending test), 층간전단실험(interlaminar shear test), 그리고 인장실험(tensile test)을 수행하였다. 골절부에 삽입된 고정판은 치료 기간 동안 반복적인 하중을 받기 때문에 제작한 실제 고정판이 충분한 피로수명을 가져야 한다. 따라서 제작한 생분해성 복합재료 고정판의 피로 수명을 평가하기 위해 유한요소해석을 통해 계산된 하중 조건을 실제 제작한 고정판에 피로하중으로 부여하여 피로 실험(fatigue test)을 진행하였다. 또한, 50°C로 유지된 PBS(phosphate buffer saline) 내에서 생분해성 복합재료의 분해 속도를 확인하였다. 실험 결과들을 바탕으로 플라즈마 처리를 통해 PGF/PLA 복합재료의 기계적 물성들이 효과적으로 개선된 것을 확인하였다.
