Objective: Bisphosphonates related with osteonecrosis of maxilla, and maxillary sinus mucosa cells (MSMCs) have the ability to differentiate into osteoblasts. However, the effects of BPs on bone metabolism in MSMCs are not completely understood. The aim of this study was to investigate the effects of BP such as zoledronic acid (ZA) and alendronate (AN) on osteoblastic and osteoclastic differentiation in MSMCs, and its related signal pathways. Methods and materials: Stem cell markers was detected by FACS analysis. The proliferation of MSMCs was determined by MTS assay. Osteoblastic differentiation was analyzed by Alizarin red S staining, RT-PCR and ALP activity assay. Osteoclastic differentiation was examined by RT-PCR,real time PCR, TRAP staining, f-actin staing and bone resorption assay. Signal pathways were evaluated by Western blot analysis. Results: MSMCs were positive for STRO-1 and negative for CD34 and CD45, and had the ability to differentiate into osteoblasts under osteogenic induction. Proliferation of MSMCs was enhanced by ZA, but decreased by AN. ZA and AN suppressed osteoblastic differentiation, as evidenced by ALP activity, mineralization nodule formation and mRNA expression of transcript factor Runx2 in dose-dependent manner. AN dose-dependently increased mRNA of RANKL/OPG ratio, whereas ZA reduced it. Consistently, conditioned medium (CM) obtained from MSMCs cultured with AN stimulated osteoclastic differentiation via transcript factor NFATc-1 signaling, but CM from ZA inhibited it. ZA and AN did not affect Wnt1 and Wnt3a, but increased translocation of β-catenin into nuclear by enhancing phosphorylation of Akt. However AN and ZA reduced the expression of Wnt5a and it’s down-stream signaling such as CaMKII. After rh-Wnt5a addition, AN and ZA increased the ostoegeneic differentiation in MSMCs, and AN declined the mRNA ratio of RNAKL and OPG in MSMCs to decrease osteoclastic differentiation, compared with only AN and ZA treatment. Conclusion: This study, for the first time, demonstrated both ZA and AN suppressed osteoblastic differentiation in MSMCs. In contrast, AN promoted osteoclastic differentiation through enhancing RANKL/OPG ratio in MSMCs, whereas ZA suppressed it. These data suggested that taking BPs may be negative factor for the bone formation in maxillary sinus for osteolytic disease patients.
Bisphosphonate(BP)는 골다공증 치료제로 광범위하게 사용되고 있지만, 상악골의 괴사 등 부작용이 보고되고 있다. 상악골에 위치한 상악동점막세포는 골모세포로 분화 된다고 밝혀져 있지만, 상악동점막세포의 골 대사 시 BP의 효과는 아직 밝혀지지 않았다. 따라서 이 연구의 목적은 상악동점막세포가 조골 및 파골로 분화 될 때, BP의 제제 중 zoledronic acid(ZA) 와 alendronate(AN)의 효과를 밝히고 또한 관련된 신호전달을 밝히고자 한다. 줄기세포 표지자는 유동세포분석법(FACS)를 통해 확인하였고, 상악동 점막세포의 증식을 MTT assay를 통해 측정하였다. 조골 분화를 확인하기 위해 ALP 활성 측정, RT-PCR, alizarin red 염색을 시행하였고, 파골 분화는 RT-PCR과 real-time PCR, TRAP 염색을 통해 확인하였다. 신호전달 단백질 활성은 western blot을 통해 분석하였다. 상악동점막세포에서 줄기세포 대표 표지자인 STRO-1이 발현되었지만, CD34와 c-kit은 발현되지 않았다. ZA에 의해 상악동 점막세포의 증식이 증가하였지만, AN에 의해 증식이 감소하였다. ALP 활성과 광물화 결정형성, 골분화 대표 전사인자인 Runx2의 발현이 ZN과 AN 농도의존적으로 감소하였다. RANKL과 OPG의 mRNA비율은 AN 농도 의존적으로 증가되었지만, ZA는 농도 의존적으로 감소되었다. 상악동점막세포에 ZN과 AN을 처리하여 획득한 배지(conditioned media)를 통해 파골 분화를 유도한 결과 ZA conditioned 배지에 의해서 파골 분화가 억제되었고, AN conditioned 배지에 의해 파골 분화가 증가되었다. 조골, 파골과 관련된 신호전달을 알아보기 위해 Wnt 신호전달을 확인하였다. ZA과 AN은 Wnt signal 중 canonical signal (Wnt1, Wnt3a)에는 영향이 없었지만, AN이 Akt(Ser473)의 활성과 β-catenin활성을 증가시켰다. 하지만, noncanonical signal인 Wnt5a와 하위 신호인자(CaMKll)의 활성이 ZA과 AN에 의해 감소하였다. 이러한 결과를 종합하여 보았을 때, ZA과 AN가 상악동점막세포의 조골 분화를 억제하였고, AN가 ZA보다 RANKL과 NFATc1를 증가시켜 noncanonical Wnt 신호전달과 Akt/β-catenin 신호전달을 통해 파골 분화 유도를 촉진하였다. 따라서 본 연구는 상악동점막세포에서 ZN과 AN의 효과를 처음으로 것명하였으며, 골용해성 질환자에서 상악동 거상술 또는 골이식 동안 ZA 과 AN의 투여가 추천되지 않는다고 사료된다.
