장기 이식을 기다리는 환자는 매년 18,000여명에 달하고 있지만 실제 장기 이식을 받는 건수는 한해 약 2,500여건으로 장기 이식에 필요한 장기가 턱없이 부족한 상황이다. 이러한 상황을 극복하기 위하여 이종간의 장기 이식에 초점을 맞추어 연구가 진행되어 왔다. 그 중에서도 특히 미니 돼지를 이용한 장기 이식에 관한 연구가 활발히 진행 되고 있다. 그 이유는 장기의 해부학적, 생리학적인 면에서 인간과 돼지는 매우 유사함을 보이고 있고 사람으로 옮길 수 있는 질병이 원숭이나 다른 동물과는 다르게 거의 없다는 점이 있다. 또한 특정 병원성 감염원에 자유로운 사육이 가능하며, 임신기간이 매우 짧고, 많은 수의 새끼를 얻을 수 있으며, 적은 비용으로 장기 이식을 위한 미니 돼지를 만들 수 있는 장점이 있다. 그러나 완벽한 이종장기 이식을 위해서는 몇 단계의 면역 거부 반응을 극복해야 한다. 초급성 면역거부반응과 세포성 면역거부반응, 만성 면역거부반응을 차례로 해결해야 한다. 본 연구는 초급성 면역거부반응과 세포성 면역거부반응을 억제하기 위하여 미니돼지의 태아섬유아세포에 보체활성을 조절하는 유전자인 human CD59와 NK cell, T-cell, macrophage의 증식 및 활성화 에 크게 관여하고 있는 IL-18의 활성을 낮출 수 있는 IL-18 binding protein a 두 개의 유전자를 도입시켜 초급성 면역거부반응과 세포성 면역 거부 반응을 억제 시킬 수 있는 세포주를 만들게 되었다. 이 세포주를 이용하여 인간의 체액을 통한 보체활성 억제에 대한 실험결과 형질전환 세포주에서는 세포의 미토콘드리아 활성에 변화가 없었지만, 일반 미니돼지의 태아섬유아세포에서는 미토콘드리아의 활성이 떨어지는 것을 확인 하였다 (p
Xenotransplantation involves multiple steps of immune rejection. The present study was designed to overcome hyperacute rejection (HAR) and cellular rejection. Human complement regulatory protein hCD59 has been known to reduce the hyperacute rejection. Additionally, IL-18BPa may prevent cellular rejection by inhibiting the activation of NK cells, activated T-cell proliferation, and induction of IFN-γ. Transgenes separately including hCD59 and ILI-18BPa were introduced into miniature pig fetal fibroblasts. Double-transgenic fibroblasts were selected by treatment of antibiotics in culture. Resulting fibroblasts contained two transgeses based on PCR, and their expression was confirmed by RT-PCR. Treatment of human serum did not affect the survival of hCD59 and IL-18BPa double-transgenic fibroblasts, whereas the treatment significantly reduced the survival of non-transgenic fibroblasts (p< 0.01). The double-transgenic fibroblasts presumably capable of alleviating HAR were transferred into enucleated oocytes. Among 337 reconstituted oocytes, 28 (15.3%) developed to the blastocyst stage. Analysis of individual embryos indicated that 53.6% (15/28) of embryos contained the transgene hCD56 and IL-18BPa. The result of the present study demonstrates the resistance of hCD59 and IL-18BPa double-transgenic fibroblasts against HAR. The usefulness of the transgenic approach may be predicted by RT-PCR and cytolytic assessment prior to actual production of transgenic pigs. Further study on the transfer of these embryos to surrogates may produce transgenic clone miniature pigs expressing hCD59 and IL-18BPa for xenotransplantation.
