본 연구는 뇌졸중 환자를 대상으로 페달링 운동을 전방과 후방으로 나누어 환자에게 미치는 영향에 대해 알아보았다. 전라남도에 소재한 J병원에서 입원한 뇌졸중환자 30명을 대상으로 전방 페달링 운동을 중재한 집단15명을 실험군Ⅰ, 후방 페달링 운동을 중재한 집단 15명을 실험군Ⅱ로 선정하여 총 6주 간 중재를 시행하였다. 실험군Ⅰ과 실험군Ⅱ의 중재 후 집단 내 근활성도 측정결과 두 집단 모두 각각 통계학적으로 유의한 차이가 있었으며(p<.05), 집단 간 비교에서도 통계학적으로 유의한차이가 있었다(p<.05). 집단 내 H-반사 측정 결과 두 집단모두 통계학적으로 유의한 차이가 있었다(p<.05). 집단내 10 m 보행 검사를 통해 보행속도 측정 결과 두 집단모두 통계학적으로 유의한 차이가 있었으며(p<.05)(p<.01), 실험군Ⅰ과 실험군Ⅱ의 각 집단별 종속변인들에 대한 상관관계는 실험군Ⅰ에서 넙다리곧은근의근활성도가 증가할수록 넙다리네갈래근의 활성도가 감소하는 음의 상관관계를 보였고, 넙다리곧은근의 근활성도가 증가할수록 H-반사가 감소하는 음의 상관관계를보였다(p<.05). 그리고 실험군Ⅱ에서는 넙다리두갈래근의 근활성도가 증가할수록 넙다리곧은근의 근활성도가감소하는 음의 상관관계를 보였고, 가자미근 활성도가증가할수록 앞정강근의 활성도가 감소하는 음의 상관관계를 보였다,(p<.05). 후방 페달링 운동은 운동 겉질 영역의 산소헤모글로빈(HbO2)의 증가로 근피로도를 감소시키기 때문에 다리의 근활성도 변화에 더 큰 영향을 미칠수 있을 것이다.
This study aimed to investigate the effects of each of the forward and backward pedaling exercises on stroke patients. The intervention was conducted on 30 stroke patients who were hospitalized at J Hospital in Jeollanam-do for a total of 6 weeks by selecting 15 ones who were subject to the intervention of a forward pedaling exercise as Experimental Group Ⅰ and 15 ones who were subject to the intervention of a backward pedaling exercise as Experimental Group Ⅱ. intervened by backward pedaling exercise for a total of 6 weeks. Lower extremity muscle activity and H-reflex were measured using Biopac MP-150, while gait speed was measured using a 10 m walking test prior to intervention. And, after a 6-week intervention, those items were re-measured and analyzed in the same way. The measurement results of muscle activity within the groups after the intervention for the Experimental Groups Ⅰ and Ⅱ showed that there was a statistically significant difference in both groups (p<.05) and in the comparison between both groups (p<.05). The measurement results of the H-reflex in the groups showed that there was a statistically significant difference in both groups (p<.05). The measurement results of gait speed through the 10-m walk test in the groups showed that there was a statistically significant difference between both groups(p<.05)(p<.01), and that there was a negative correlation for dependent variables in both of the Experimental Groups Ⅰ and Ⅱ (p<.05). It is believed that the backward pedaling exercise may have a greater effect on the change in the muscle activity of legs as it reduces muscle fatigue, thereby increasing oxyhemoglobin (HbO2) in the motor cortex
