Primary and secondary hypnic headaches (HH) are very rare forms of headache from the group of secondary headaches (International Classification of Headache Disorders). The 55-year-old woman presented with morning headaches typical of HH, but also showed other symptoms indicative of secondary HH (hypertension and obstructive sleep apnea syndrome [OSAS]). The cause of HH was diagnosed as OSAS. After clarification in a sleep laboratory, the patient was fitted with a sleep mask and her blood pressure was optimized so that the symptoms almost went into complete remission. To optimize the circadian rhythm and sleep, treatment with melatonin was also successfully started.
