1967년 1월부터 1981년 12월까지 15년간 일신기독병원에서 사망한 임산부 221예를 대상으로 5년씩 3기간으로 나누어 (1967∼71, 1972∼76, 1977∼81) 분석하였다. 이에 대한 결과를 보면 다음과 같다. 1. 도착사를 제외한 모성사망율이 총 출산아 10,000명당 55.5에서 30.9로 최근 5년간은 9.2로 현저히 감소되었다. 도착사 및 유산 또는 산후에 입원하여 사망한 환자를 제외한 교정된 병원모성사망율은 31.4에서 20.7로, 최근 5년간은 5.9로 감소되었다. 2. 모성사망의 주요원인을 분석해 보면 1) 태아 생존가능 이전의 (임신 20주이하) 사망원인은 첫 두기간동안 주로 패혈성 인공유산이었다. 그러나 최근 5년간은 없었다. 2) 태아 생존가능 이후, 직접적인 산과적 사망은 59.0%→6.4%→79.4%로 증가했지만 간접적인 산과적 사망은 41.0%→38.6%→20.6%로 감소되었다. 3) 15년간 간접적인 모성사망의 주요원인은 뇌출혈 및 고혈압성 심장질환이었다. 4) 직접적인 산과적 사망의 주요원인은 임신중독증, 출혈 감염사 등이었지만 이 중 임신중독증에 의한 사망이 가장 많았고 그 비율은 55.8%→52.0%→50.0%로 약간 감소되었을 뿐이며 출혈에 의한 사망은 26.9%→38.0%→38.5%로 오히려 증가되는 추세였다. 감염사로 인한 사망은 13.5%→8.0%→7.7%로 약간 저하되었지만 실제로 큰 변화는 없었다. 3. 모성사망율이 감소된 이유를 분석해 보면 1) 연령 및 분만회수를 볼때 고위험군에 속하는 수가 많이 줄었다는 것이다. 즉 35세 이상이 9.0%→6.1%→3.8%로, 분만회수 4이상의 경산부가 7.9%→4.5%→2.5%로 감소되었다. 2) 산전관리가 상당히 큰 영향을 미치는 것 같다. 전 15년동안 산전진찰을 받은 군의 모성사망율이 2.2인데 비하여 산전진찰을 받지 않은 군의 사망율은 61.2로 월등히 높았다. 1967년에 산전관리를 받은 임산부는 40%에 불과했으나 1981년에는 75%로 증가되었으며 이는 동기간중 모성사망율을 83.0에서 1.3으로 급격히 저하시킨 중요한 원인이라고 생각된다. 4. 모성사망율을 더욱 더 감소시키기 위해서 철저한 산전관리와 이로인한 병원 분만수의 증가이며 다음은 고위험을 임신연령군에 대한 조사와 치료강화이고 정확한 사인을 규명하기 위해서 부검을 권장해야 될 것이다.
In order to further reduce maternal mortality the major causes of death and avoidable factors much furst be known, and the results of such an analysis of 221 maternal deaths occrrring at Il Sin Women`s Hospital, Pusan, between 1967∼1981 are presented. The results are presented in three 5 years periods. 1. The overall maternal mortality rate decreased successively from 55.5 to 30.9 to 9.2 per 10,000 total births. The corrected hospital maternal mortality rate, that is in which patients admitted postabortal or postpartum were excluded, decreased from 31.4 to 20.7 to 5.9 per 10,000 total births. 2. The major causes of death were as follows, (1) In the previable period of pregnancy, that is less than 20 weeks gestation, septic induced abortion was the main cause in the first two periods but no such death occurred in the last 5 years. (2) In the viable period of pregnancy the percentage of direct obstetric deaths has risen from 59.0% to 79.4% as that of indirect obstetric deaths decreased from 41.0% to 26.0%. (3) Cerebral hemorrhage and hypertensive heart disease have remained as the main causes of indirect obstetric deaths over the 15 years. (4) The main causes of direct obstetric deaths were toxemia, hemorrhage and sepsis and although the incidence of all have decreased the relative proportions have remained much the same. The percentage of deaths due to eclampsia or preeclampsia decreased from 55.8% to 50.0% an that of deaths due to hemorrhage increased from 26.9% to 38.5%. 3. Possible reasons for the decrease in maternal mortality rate. (1) There has been a marked decrease in the number of women who are in the high risk group according to their age or parity, that is the percentage of those aged 35 or more decreased from 9.0% to 3.8% and the percentage of para 4 or more decreased from 7.9% to 2.5% (2) M.M.R. is directly related to the amount of antenatal care received and the overall rate for the emergency admissions over the 15 years was 61.2 per 10,000 total births compared to 2.2 in those receiving antenatal care. The increase in percentage of those attending for antenatal care has risen from 40% in 1967 to 75% in 1981 and this is considered to be a major cause for the corresponding decrease in M.M.R. from 83.0 to 1,3. 4. Possible methods for further reducing the maternal mortality rate. (1) Increased emphasis needed on the importance of antenatal care. (2) Earlier admission would have prevented some deaths as 30% of the patients died within 6 hours of admission. This was mainly due to the patient`s negligence but also late transfer without adequate resuscitation by local doctors and midwives was a factor and establishment of emergency services attached to major ovstetric hospitals which could go out to those patients and transport them safely would further reduce the mortality.
