본 연구의 목적은 중등도 이상의 뇌성마비아의 의사소통 능력 감별척도 예비본을 개발하고 전문가를 대상으로 타당도를 검증하는 것이다. 이를 위해 국·내외 문헌을 조사, 분석하여 초기 문항을 도출 후 관련분야의 전문가 패널을 대상으로 1·2차 델파이 조사를 통하여 말·언어 능력 평가 항목들에 대한 내용타당도를 확보하였다. 호흡, 발성, 구강구조 기능 및 조음, 언어 영역의 하위문항을 포함하고 있는 뇌성마비아 의사소통능력 감별척도는 내용타당도 비율의 값이 적절함으로 나타났다. 개발된 척도를 실제 사례에 적용해본 결과, 중증도가 심하여 공식검사에서는 파악하기 어려웠던 수용, 표현 의사소통능력과 구어메커니즘에 대한 상세한 프로파일화가 가능하였다. 이를 통해, 중증도 이상의 뇌성마비아의 개별화 치료 전략을 개발하고 아동의 수준에 적합한 양질의 언어치료 교육이 가능해질 것으로 기대한다.
The purpose of this study is to develop a preliminary model of the communication ability differential scale for children with moderate to severe cerebral palsy and to verify its validity targeting experts. To this end, domestic and foreign literature was investigated and analyzed to derive initial questions, and then content validity was secured for speech and language ability evaluation items through the 1st and 2nd Delphi surveys targeting expert panels in related fields. The value of the content validity ratio was found to be appropriate for the cerebral palsy communication ability differential scale, which includes the sub-items of breathing, vocalization, oral structure function, articulation, and language. As a result of applying the developed scale to actual cases, detailed profiling of receptive, expressive, and communication skills and spoken language mechanisms, which were difficult to grasp in official tests due to their severe, was possible. Through this, it is expected that an individualized treatment strategy for cerebral palsy of more than moderate severity will be developed and quality speech therapy education suitable for the child’s level will be possible.
