연구목적: 대부분 의료기관은 병원정보시스템을 운영하며 다양한 목적을 위해 응급환자의 정보를 요약하여 제공한다. 이 요약 정보를 이용하여 응급실의 수행능력을 평가하고 전국의 모든 응급환자의 정보가 요약된데이터베이스를 이용하여 국가 외상 치료 프로그램을 분석하고자 한다.연구방법: 이 연구는 세 가지로 분류되는 후향적 관찰 연구이다. 첫째, 응급실의 협진 및 입원 결정 체계를 바꾸어 응급실 체류 시간의 감소가있었는지 살펴보았다. 두 번째로 단일 대학병원에서 일시적인 심각한의료진 부족의 위기에서 변형된 시스템을 적용하고 운용하였으며 이결과를 평소의 응급실 운영과 비교하였다. 결과 변수는 응급실 체류시간, 사망률, 병원 사망률 등으로 추출하였으며 로지스틱 회귀분석을통해 분석하였다. 셋째로 NEDIS 자료를 이용하여 외상 센터와응급의료센터 간의 외상 환자 사망률을 중증도를 보정 후 비교하였다. 또한, 수상 기전 및 수상 부위의 차이가 두 군 간의 사망률 차이에영향을 미치는지 분석하였다. 연구결과: 입원 결정 체계의 수정 후 내과 입원환자의 응급실 체류 시간중간값은 673 분 (IQR: 347—1,369)에서 237 분 (IQR:166—364)으로 감소하였다. 파업 기간 응급의학과 전문의에 의한 직접진료가 협진 수는 적었으나 검사실 검사 및 영상 검사의 수는 줄이지못하였다. 해당 기간의 응급실 체류 시간은 359 분(IQR: 147–391)에서 326 분(IQR: 123–318)으로 통계적으로 의미 있게 감소하였다.(P
Backgrounds: Assessing the performance of emergency medical system or emergency center was necessary to set up policy plan and operate the system efficiently. We tried to evaluate the performance of the emergency department (ED) of the study hospital using the summary data and analyze the national trauma care program using a national database of emergency patient care summary. Methods: The studies were three serial retrospective observational studies. Firstly, comparing the ED length of stay (EDLOS) of patients admitted to the internal medicine (IM) department before and after the policy change regarding admission. The EDLOS and other indicators of patients admitted to the IM department were compared. The second study evaluated ED flow and performance during the junior doctors’ strike and compared it with the usual period. The outcome variables were defined as EDLOS, mortality, and adjusted for demographic and clinical parameters and analyzed. The last study evaluated the difference of propensity-score matched mortality between trauma center (TC) and emergency center (EC). Results: The median EDLOS of patients admitted to the IM department decreased from 673 (interquartile range, IQR: 347—1,369) minutes in the control period to 237 (IQR: 166—364) minutes in the first study. During the trainee doctors’ strike, emergency specialists managed patients with fewer consultations. However, the proportion of patients who underwent laboratory and radiological tests did not change significantly. The median EDLOS significantly decreased from 359 minutes (IQR: 147–391) in the control period to 326 minutes (IQR: 123–318) during the strike period. (P
