목적: 오토파지는 영양막세포의 침윤성에 영향을 미치는 것으로 생각되고 있으며, 오토파지를 억제했을 때 영양막세포주의 침윤이 증가한 것을 이전 연구에서 밝혀진 바 있다. 하이드록시클로로퀸은 다양한 작용을 가진 항말라리아약제로 오토파지를 억제하는 작용도 가지고 있으며, 산과적 합병증을 예방하는데 있어서 그 기전과 효과에 대한 연구들이 이루어지고 있다. 본 연구에서는 하이드록시클로로퀸의 오토파지를 통한 영양막세포 침윤에 미치는 영향에 대해 알아보고자 한다.방법: 제 1삼분기 영양막세포주인 HTR-8/SVneo 세포를 이용하여 메트리겔 침공 분석 검사를 시행했다. 오토파지 유도를 위해 라파마이신 (1 ㎛)을 처리하고 오토파지 억제를 위해 하이드록시클로로퀸 (20 ㎛)을 추가로 처리했다. 하이드록시클로로퀸의 오토파지를 통한 영양막세포 침윤에 연관된 단백을 확인하기 위해 효소결합면역흡착 분석을 통해 soluble fms-like tyrosine kinase-1 (sFlt-1), placental growth factor (PlGF), matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9), IL-6, endothelin-1 (ET-1)의 양을 확인했다. 영양막세포 침윤의 기전을 확인하기 위해 웨스턴 블랏으로 extracellular signal-regulated kinase (ERK), signal transducer, activator of transcription 3 (STAT3)를 확인했다. 또한 ERK 억제제를 사용했을 때 영양막 세포 침윤의 변화를 확인하였다. 결과: 하이드록시클로로퀸을 처리했을 때 LC3, p62 단백이 축적되는 것을 확인했고, 이는 HTR-8/SVneo 세포에서 오토파지가 억제되었음을 의미한다. 침공 분석에서는 라파마이신 처리로 인해 감소했던 영양막세포의 침윤성이 하이드록시클로로퀸을 처리함으로서 다시 회복되는 것을 확인할 수 있었다. 라파마이신과 하이드록시클로로퀸을 함께 처리했을 때 라파마이신만 처리한 세포에 비해서, 영양막세포 침윤과 관련된 단백 중 MMP-2, MMP-9는 변화가 없었고, sFlt-1/PlGF는 오히려 증가한 것을 확인할 수 있었다. 그러나 IL-6는 증가했고, ET-1은 감소한 것이 관찰되었다. 오토파지와 관련된 영양막세포의 침윤 기전을 확인하기 위해 라파마이신과 하이드록시클로로퀸을 함께 처리한 세포에서 인산화 ERK가 유의하게 증가한 것이 확인되었고, ERK 억제제를 처리했을 때 인산화 ERK는 감소했으나 인산화 STAT3는 변화가 없었다. 결론: 본 연구를 통해 하이드록시클로로퀸이 오토파지 억제를 통해 ERK 신호체계를 활성화하여 IL-6와 ET-1에 영향을 줌으로써 영양막세포의 침윤성을 증가시킨다는 것을 확인하였다.
Objective: Hydroxychloroquine (HCQ), which has multiple actions including inhibition of autophagy, is increasing in use for prevention of obstetric complications such as preeclampsia in women with lupus and anti-phospholipid antibody syndrome. However, exact mechanism of action of HCQ in placentation is not clearly elucidated. Here, we aimed to investigate the in vitro effect of HCQ on trophoblast invasion in relation to autophagy.Methods: We used HTR-8/SVneo cells, a commonly used extravillous trophoblastic (EVT) cell line, and performed a matrigel invasion assay. Cells were treated with rapamycin to induce autophagy with or without HCQ. We checked autophagy-related proteins (p62, LC3-II) by Western blot and confocal microscopy and measured several proteins related to trophoblast invasion including soluble fms-like tyrosine kinase-1 (sFlt-1), placental growth factor (PlGF), matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9), IL-6 and endothelin-1 (ET-1) by ELISA. We also measured extracellular signal-regulated kinase (ERK), signal transducer and activator of transcription 3 (STAT3) by western blot to understand the signaling pathways related trophoblastic invasion. We used PD98059, ERK inhibitor, to check whether the treatment of ERK inhibitor affect trophoblast invasion by co-treatment of HCQ.Results: HCQ increased the expression of LC3-II and p62 protein and decreased LC3 puncta with less co-localization with LAMP2 compared to rapamycin only treatment, indicating inhibition of autophagy by HCQ in HTR-8/SVneo cells. HCQ restored rapamycin-induced decreased trophoblast invasion. Of note, HCQ co-treatment increased level of IL-6, and decreased level of ET-1 compared to rapamycin only treatment. HCQ did not affect the levels of MMP-2 and MMP-9 according to autophagy induction by rapamycin and inhibition by HCQ. Being different from expectation, HCQ increased sFlt-1/PlGF compared to rapamycin only treatment in in HTR-8/SVneo cells. For signaling pathway, HCQ increased the level of phosphorylated ERK and phosphorylated STAT 3 compared to rapamycin only treatment. Finally, as an inhibitor test, the treatment of ERK inhibitor affected to lower trophoblast invasion. Conclusion: Our data indicated that HCQ could improve trophoblast invasion by affecting IL-6 and ET-1 level via ERK signaling pathway.
