Background: The chemokine CXCL13 is known to influence local anti-tumor immunity by recruiting immune cells and forming tertiary lymphoid structures (TLS). It has been hypothesized that TLS, led by the expression of CXCL13, could be a predictive or prognostic biomarker to immunotherapy by its involvement in priming the adaptive anti-tumor response. Methods: Based on the TCGA data showing a positive correlation between TLS and the CXCL13 expression, we constructed an exploratory dataset (n =154) with the lung adenocarcinoma samples available for the clinical response to the immune checkpoint inhibitor (ICI). Clinical impact and correlation to the immune response-related genes or gene sets were analyzed. Outcomes from the exploratory set were validated through the public database (n=43) and compared with the single-cell RNA sequencing (scRNA-seq) result from another cohort (n=9, 31743 cells)Results: From the differentially expressed gene analyses, CXCL13 expression was significantly upregulated in the responders (p = 0.001) along with other cytolytic activity-related genes, CD8B, IFNG. Survival analysis using a cut-off value of the median expression value of CXCL13 showed prolonged progression-free survival (p < 0.001) and overall survival (p = 0.001). CXCL13 expression was higher in the case of PD-L1 protein expression ≥ 50% (p = 0.004). Notably, subgroup analyses based on CXCL13 expression and CD8A showed that CXCL13hi/CD8Ahi patients demonstrated prolonged survival compared to the CXCL13low/CD8Ahi patients (p = 0.006). From the scRNA-seq data, CXCL13 expression was majorly observed in CD8 T-cell, showing a similar expression pattern with other inhibitor immune checkpoints, PD-1, TIGIIT, TIM3, LAG3.Conclusion: Our data showed prolonged PFS to ICI in CXCL13 upregulated patients. Also, CXCL13 showed a positive correlation to cytolytic activity and inhibitory immune checkpoint related genes and TLS-related gene sets. This finding suggests that CXCL13 production might be an essential component in reinvigorating the anti-tumor immunity by ICI.
사이토카인 CXCL13 은 다양한 면역 세포들을 유인하고 tertiary lymphoid structure (TLS) 구조를 형성함으로써 국소적인 항암 면역작용을 일으키는 것으로 알려짐. 특히 TLS는 CXCL13 의 발현에 의해 형성이 되고, 적응성 면역체계의 활성화를 통해 면역항암제의 치료 반응과 예후에 영향을 줄 것이라고 가정됨. TCGA 자료를 통해 TLS 와 CXCL13의 발현이 상관 관계가 있음을 확인 후, 총 154명의 면역항암제 치료를 받은 폐 선암 환자로 구성된 탐색 데이터세트를 구성함. 임상적인 유용성과 다른 면역작용 관련 유전자와의 상관 관계의 분석을 시행함. 탐색 데이터세트에서의 결과는 외부 데이터세트를 통해 검증을 하였고, 본 연구자가 기 확보한 단일세포유전체 분석 결과 (n=9, 31743 세포) 와 비교하여 분석함. 탐색 데이터세트에서 유전체 발현 자료를 비교한 결과 CXCL13 은 면역 관문억제제에 반응을 했던 환자들에서 통계적으로 유의하게 상승함이 관찰되었고 (p = 0.001) 세포독성 반응과 연관성이 알려진 CD8, IFNG 와 상관성을 보임. CXCL13 발현의 중앙값을 이용하여 생존분석을 시행 하였을 때 무진행 생존기간 (p < 0.001) 및 최종생존기간 모두 의미 있는 차이를 보임 (p = 0.001). CXCL13의 발현은 PD-L1 단백질이 50% 이상 발현된 환자군에서 높은 양상으로 관찰됨 (p = 0.004). 실제로, CXCL13 과 CD8A의 발현양상을 비교하였을 때 CD8A 가 높은 환자에서 CXCL13의 발현이 높은 환자가 CXCL13 의 발현이 낮은 환자보다 더 긴 생존기간을 보임 (p = 0.006). 단일세포 유전체 분석 결과, CXCL13 은 CD8 T-cell에서 주로 발현이 되는 것을 확인할 수 있었고, 다른 PD-1, TIGIT, TIM3, LAG3 와 같은 억제성 면역관문 유전자의 발현과 연관이 있음을 확인함. 본 연구의 결과를 통해 CXCL13의 발현이 높은 환자에서 면역 관문억제제의 반응이 좋음을 확인함. 또한 CXCL13의 발현은 세포독성 및 억제선 면역관문 유전자, TLS와 연관된 유전자의 발현과 연관이 있음을 확임 함. 본 연구의 결과를 통해 CXCL13 이 면역 관문억제제에 의해 종양 미세환경의 변화를 일으킬 수 있는 주요한 예측 인자로서의 가능성을 확인함.
