경두개전기자극(transcranial electrical stimulation: 이하 tES)는 대뇌 흥분성을 조절하기 위해 두피에 전류를 인가하는 신기술이다. tES는 두가지 방법으로 구분된다: 경두개 직류/교류 자극 (transcranial DC/AC stimulation: 이하 tDCS/tACS). 본 연구에서는 tES가 특정 주파수에 맞춰 뉴런의 활동성을 변조시킬 수 있는가를 조사하였다. 이를 위해, 특정 주파수로 깜빡이는 시각 자극을 이용하여 뉴런을 동기화 시키는 안정 상태 시각 유발 전위 (steady-state visual evoked potential: 이하 SSVEP)를 사용하였다. 본 연구에는 그룹별로 10명의 피험자가 참가하였다: tDCS, tACS 그룹. tES 자극을 받기 전후로 20 Hz 시각자극을 6초씩 20번 반복해서 제시하였고 (2 세션), 시각자극을 보는 동안 두피 전체에 부착한 24개의 전극을 이용하여 뇌파 데이터를 수집하였다. tDCS, tACS 그리고 sham 조건에 대해 각각 15분씩 자극을 인가하였고, 두 세션은 동일한 비율로 구분하였다. 특히, SSVEP와 tACS 자극 간의 주파수를 일치시키기 위해 tACS 자극을 20Hz로 인가하였다. 이동-창 기술을 이용하여 각각의 시행에 대해 파워 스펙트럼 밀도 (PSD)를 계산하고, 20 Hz의 PSD를 추출하여 tES에 의해 20 Hz에 상응하는 뇌 활동성이 증가하는지 확인하였다. tDCS와 tACS 그룹 모두 두피 전 영역의 20 Hz 파워가 유의미하게 증가하는 것을 확인하였다. 반면, tDCS와 tACS 그룹의 sham 조건에서는 모두 유의미한 뇌 활동 변화를 확인할 수 없었다. 시각 정보 처리를 관장하는 후두엽에서는 tES 자극 전과 후를 비교하였을 때, tACS 그룹에서만 20 Hz의 뇌 활동 변화가 유의미하게 증가하는 것을 관찰하였다 (p < 0.001). 결과적으로 자극을 인가하면 시각을 관장하는 영역에서 20 Hz tACS 효과가 강하게 나타났다. 그러므로, 본 연구에서는 특정 주파수에 맞춰 뉴런의 활동성을 변조시키기 위해 두 자극 (시각, 뇌 자극) 간의 동기화 가능성을 확인하였다.
Transcranial electrical stimulation (tES) is an emerging technique that applies an electrical current to the scalp to modulate cortical excitability. There are two types of tES methods: transcranial DC/AC stimulation (tDCS/tACS). In this thesis, I investigated whether tES can modulate frequency-specific neural oscillation. To this end, I employed steady-state visual evoked potential (SSVEP) that is a brain response elicited by the repetitive presentation of visual stimuli with a certain frequency. Two groups of ten subjects were recruited for this study: tDCS and tACS group. In the experiment, a 20 Hz visual stimulus was presented for 6s and 20 times before and after tES (two sessions), during which electroencephalography (EEG) was measured using 24 channels broadly attached to the scalp. tDCS and tACS along with SHAM condition were applied to tDCS and tACS group for 15 min, counterbalanced, respectively, between the two sessions. In particular, a 20 Hz current was used for tACS to match the stimulation frequencies of SSVEP and tACS. Power spectral density (PSD) was calculated for each trial using a moving window technique, and PSDs at 20 Hz was extracted in order to check whether tES increases 20 Hz brain activity. Both the tDCS and tACS group showed significantly increased spectral power at 20 Hz after tES when the whole brain was analyzed. On the other hand, there were no significant brain activity changes in the sham condition for both tDCS and tACS group. In the occipital brain area that is responsible for visual information processing, a significant increase in 20 Hz brain activity was only observed for tACS group after tES as compared to before tES (p < 0.001). The result indicates a strong impact of 20 Hz tACS on the visual region of the brain after the stimulation applied to the subjects. For this reason, we confirmed the feasibility of synchronization between two different stimuli (visual and brain stimulation) to modulate neural activity in a specific frequency.
