Objectives: Tuberculosis (TB) is among the deadest infectious disease with a high global burden. It is necessary to discover and validate robust blood-based biomarkers to improve the management of TB. The host immune system contributes effectively to pathogen Mycobacterium tuberculosis (Mtb) eradication. Notably, cytokines play a critical role in the host immune responses to Mtb infection. The study aimed at discovering the most potential cytokine-related biomarkers to be used in TB treatment monitoring, diagnosis, outcome prediction, and progression risk examination. Methods: In this study, the immune biomarker candidates were collected from available knowledge-based resources. A robust transcript biosignature with significant kinetic alterations during TB treatment was then established via the data-driven procedure. The capacity in TB management of the biosignature was assessed using tree-based machine learning models (Decision Tree, Random Forest, eXtreme Gradient Boosting) and gene-set variation analysis in several cohorts.Results: Twelve cytokine-related genes (IFNGR2, IL-1β, IL-16, IFNAR2, CCR3, PPBP, TGFBR1, FAS, ACVR1, IRF2, GDF11, ACVR1B) were found to express differently across the time course of TB treatment. Among them, IL-1β, IFNGR2, and TGFBR1 appeared to be the most promising biomarkers after validation in different data sets. The biosignature exhibited prospect in diagnosis between TB/healthy control and active TB/ latent TB infection assessment (in most cases, AUC ≥ 0.80). Moreover, the biosignature holds potential in the treatment outcome prediction. However, the biosignature displayed weak capacity in TB progression risk examination. Conclusion: The 12-gene transcriptomics signature that reflected the kinetic change of TB treatment was found and partially validated. It holds promising capabilities in a broad spectrum of TB management, which should be further validated in subsequent investigations.
목적: 결핵은 세계적으로 높은 질병 부담을 가진 가장 치명적인 전염병 중 하나이다. 따라서 결핵을 성공적으로 관리하기 위해서는 혈액 기반 생체 지표를 활용하는 것이 효과적이다. 숙주의 면역 체계는 결핵균의 병원체 박멸에 효과적으로 기여하는데, 특히 시토카인은 결핵균 전염에 대한 숙주 면역 반응에 중요한 역할을 한다. 이에 본 연구는, 결핵 치료 모니터링, 진단, 예후 예측, 병의 진행 위험 평가에 활용하기 위한 시토카인과 관련된 전사 생체 지표 중 가장 잠재력 있는 후보군을 탐색하고자 한다.방법: 본 연구에서는 지식 기반의 데이터베이스를 활용하여 면역 생체 지표 후보군을 선정하였다. 이 후보군에서 데이터 기반 분석을 통해 결핵 치료 기간 중 유의한 동적 변화를 보인 로버스트한 전사 지표를 최종 선정하였다. 해당 지표의 결핵 관리 능력을 평가하기 위해 다양한 임상 코호트를 활용하여 나무-기반 모형과 유전자 집합 변동 분석을 수행하였다.결과: 12개의 시토카인 관련 유전자(IFNGR2, IL-1β, IL-16, IFNAR2, CCR3, PPBP, TGFBR1, FAS, ACVR1, IRF2, GDF11, ACVR1B)가 결핵 치료 과정에서 상이하게 발현되는 것을 확인하였다. 특히 IL-1β는 결핵 감염에 관련된 숙주 저항 메커니즘으로 잘 알려진 유전자이다. 12개 생체 지표는 결핵 환자군과 정상군 혹은 활동 결핵군과 잠복 결핵 감염군을 구분하고 진단하는데 있어 충분한 잠재성을 보여주었다 (많은 경우, AUC ≥ 0.80). 더욱이, 시토카인 관련 생체 지표는, 결핵 치료의 결과를 예측하고 병의 진행 위험도를 평가할 때 우수한 성능을 보였다.결론: 결핵 치료에서 발생하는 동적 변화를 반영하여 12개의 전사체 지표를 선택하였다. 추후 연구에서 해당 지표의 광범위한 결핵 관리 능력을 살펴볼 필요가 있다.
