2형 당뇨 환자의 유병률은 국내외로 계속해서 증가하는 추세를 보이고 있다. 약물 요법은 가장 효과적인 당뇨 관리법으로 알려져 있는데, 높은 복약 순응도는 당뇨 환자의 입원률, 사망률, 합병증 발생을 감소시키며, 의료비용 절감효과도 가져온다. 하지만 2형 당뇨환자의 복약 순응도는 대부분 80%를 넘기지 못하고 있다. 모바일 앱을 활용하면 효과적으로 복약관리를 할 수 있는데, 모바일로 중재 가능한 2형 당뇨 환자의 복약 순응도 저해 요소 7가지의 중재에 집중할 필요가 있다. 본 연구에서는 지금까지 출시된 2형 당뇨 복약관리 앱들을 입체적으로 분석하고, 발전 방향에 대해서 고찰해 보고자 한다.구글 플레이 스토어에서 선별, 제외 기준을 적용하여, 분석 대상 앱들을 1차 선정하였다. 그리고 7가지 복약 순응도 저해 요소에 대한 중재 기능 포함 개수, K-MARS 점수에 따라 최종 분석 대상 앱 10개를 선정하였다. 이러한 과정은 효과적으로 복약 순응도를 향상시킬 가능성이 높은 양질의 앱을 선별하는데 초점을 맞춰서 설계되었다. 최종 선정된 10개 앱에 대해서는 상세 보유 기능, 복약 순응도 저해 요인 중재에 사용되는 기능, 사용자 리뷰 측면에서 심층 분석하였다. 1차로 99개의 앱이 선정되었고, 최종적으로 10개 앱(해외 앱 6개, 국내 앱 4개)이 선정되었다. 10개 앱이 가진 복약 기능은 총 16종으로 분석되었고, 이 중 14종이 2형 당뇨환자의 7가지 복약 순응도 저해 요인 중 최소 한 가지 이상을 중재할 수 있었다. 가장 많은 앱이 지닌 기능은 복약 시간 알림 기능이었고, 가장 적은 앱이 지닌 기능은 마음 건강 관리 기능이었다. 10개 앱 기능들에 대해 BCTs Taxonomy 및 건강 행동이론을 바탕으로 타당성을 고찰한 결과, 2형 당뇨 환자의 7가지 복약 순응도 저해 요인(우울감, 낮은 자기 효능감, 건강 지식 및 정보 부족, 단순 망각, 약물 부작용 경험, 의료진의 의사소통 능력 부족, 사회적 지지 부족)을 효과적으로 중재하기 위해 고려될 수 있는 앱 기능을 발전 방안까지 포함해서 총 16종으로 정리할 수 있었다. 향후 2형 당뇨환자를 위한 복약관리 앱 기능을 새로이 설계한다면 ‘7가지 복약 순응도 저해 요인’을 두루 중재할 수 있도록 설계하는 것이 바람직할 텐데, 그 때 이 16종의 앱 기능들을 참고해 볼 수 있을 것이다. 나아가 그 중 복약 불순응 요인 중 압도적으로 큰 비율을 차지하는 단순 망각을 중재해 주는 기능과, 7가지 복약 불순응 요인을 한 번에 두 개 이상 중재할 수 있는 기능이 핵심 기능이 될 수 있으므로 도입에 있어 우선적으로 고려할 수 있다.다만, 그 전에 본 연구 결과를 바탕으로 한 복약 앱 설계가 실제로 2형 당뇨 환자 복약 순응도를 향상시키는 데 효과가 있는지를 검증하는 후속 연구가 반드시 필요하다.
The prevalence of type 2 diabetes among patients is continuously increasing both domestically and internationally, but when it comes to diabetes management, the most critical aspect, medication adherence, is mostly below 80%. Mobile apps can be effectively used for medication management, with a focus on seven intervention factors that hinder medication adherence in type 2 diabetes patients. In this study, existing type 2 diabetes medication management apps will be comprehensively analyzed, and their future development will be considered. Health behavior theories and behavior change techniques will serve as essential foundations for designing medication management apps and will be considered together.Initially, the research subjects were screened using appropriate selection criteria from the Google Play Store. Subsequently, 10 apps were selected in a stepwise manner, which had a high potential for effectively improving medication adherence. The selected apps will then be subject to an in-depth analysis of their features, intervention capabilities related to the seven hindrances in type 2 diabetes patients' medication adherence, and user reviews.In the screening process, a total of 99 apps were selected. Among the final 10 chosen apps, they collectively had 14 meaningful medication-related features. The most common feature among the apps was a medication reminder, while the least common was mental health management. Through the analysis and review of commercially available apps, 16 specific features that medication management apps for type 2 diabetes patients should possess were identified. Each of these features falls into one or more of the following seven categories: alleviating depression and stress, boosting self-efficacy, providing health knowledge, intervening in cases of simple forgetfulness, addressing non-adherence due to drug side effects, improving communication with healthcare professionals, and offering social support.Through this study, insights into the trends in medication management apps for type 2 diabetes patients currently available in the market were gained. Additionally, by understanding the behavior change techniques and health behavior theories that underpin the design of these app features, the validity of each of the app features was assessed. For future designs of medication management apps for type 2 diabetes patients, it is desirable to incorporate features that address all seven mentioned intervention factors. Furthermore, features capable of addressing more than two of these factors simultaneously should be prioritized for more efficiency.
