Introduction: Pivot shifts occur in up to 20% of patients who undergo ACL reconstruction, and residual rotational instability is correlated with persistent complaints of subjective instability and early osteoarthritic progression. The anterolateral ligament (ALL) and the posterior menisco-capsular structure are highlighted as secondary stabilizers of a pivot shift. However, the main cause of high-grade pivot shifts is unknown. The purpose of this study was to determine the main cause of the high-grade pivot shifts in ACL-deficient knees through cadaveric experiments. Methods: In a randomized controlled trial, 10 fresh cadavers (20 knees) without anterior-posterior and rotational instability were included. Each extremity was assigned to one of 2 groups. Extremities in one group (Group 1) were released in the order of ACL and ALL. Extremities in the other group (Group 2) were released in the order of the ACL, posterolateral menisco-capsular structure (PLMCS), and posteromedial menisco-capsular structure (PMMCS). The primary outcome was the change in tibial internal rotation (IR) when 5 Nm IR torque was applied with 8.9 N valgus force. The secondary outcomes were the International Knee Documentation Committee (IKDC) grade in the pivot shift test and the incidence of the grade III pivot shift.Results: The change in tibial IR at 0° of knee flexion was not significantly increased after ALL release (1.4 ± 1.8°, p=0.052), but it was significantly increased after PLMCS (2.5 ± 1.5°, p=0.007) and PMMCS release (4.1 ± 2.3°, p=0.017). The mean IKDC grade in the pivot shift test was 2.1 ± 0.3 in extremities in Group 1 and 2.9 ± 0.3 in extremities in Group 2. The incidence of grade III pivot shifts was 10% in extremities in Group 1 and 90% in extremities in Group 2.Conclusion: In this trial, the main cause of the high-grade pivot shifts was posterior menisco-capsular structure laxity, and the role of the ALL in these shifts was limited. Preventing secondary menisco-capsular laxity by early ACL reconstruction with aggressive repair for the damaged posterior menisco-capsular junctional structure might be the most important treatment for restoring normal rotational stability.
서론 (Inroduction): 전외측 인대(ALL)와 후방 반월상연골 구조는 피벗 시프트 (Pivot shift)를 막는 이차적 안정화 구조체로 최근 관심이 집중되고 있다. 그러나 고등급의 피벗 시프트의 주된 원인은 알려져 있지 않다. 본 연구의 목적은 고등급의 피벗 시프트를 일으키는 주된 원인에 대해 카데바 실험을 통하여 확인하고자 하였다.방법 (Material & method): 전후방 및 회전 불안정성이 없는 신선 냉동 사체 10 구 (무릎=20)이 무작위 대조 시험(Randomized-control trial)에서 포함되었다. 양측 하지는 2 개의 그룹으로 배정하였으며 그룹 1 에서는 전방십자인대 (ACL), 전외측인대 (ALL) 순으로 해부학적 구조를 절제하였다. 그룹 2 에서는 전방십자인대 (ACL), 후외측 반월상 연골 구조 (Posterolateral menisco-capsular structure), 후내측 반월상 연골 구조 (Posteromedial menisco=capsular structure)의 순서로 절제하였다. 1 차적 결과 (Primary outcome)는 8.9N 외반력과 5Nm 내회전 (internal rotation) 토크를 가했을 때의 경골의 내회전 변화였으며, 2 차 결과 (Secondary outcome)는 피벗 시프트 테스트의 평균 ICRS 등급과 III 등급의 피벗 시프트의 발생률 (Incidence)이었다.결과: 슬관절 완전 신전 상태에서 경골 내회전의 변화는 전외측인대 절제 (1.4 ± 1.8°, p=0.052) 후 유의하게 증가하지 않았지만 후외측 반월상 연골 구조 (2.5 ± 1.5°, p=0.007) 및 후내측 반월상 연골 구조 (4.1 ± 2.3°, p=0.017) 절제 후에는 유의하게 증가했다. 피벗 시프트 검사시 평균 ICRS 등급은 그룹 1 에서 2.1±0.3, 그룹 2 에서 2.9±0.3 이었다. ICRS III 등급의 피벗 시프트의 발생률은 그룹 1 에서 10%, 그룹 2 에서 90%였다.결론: 본 연구에서 고등급 피벗 시프트의 주된 원인은 후방 반월판-낭 구조 이완이었으며, 전외측인대의 역할은 제한적이었다. 본 결과를 바탕으로 조기 전방십자인대 재건술과 적극적인 반월상 연골 봉합술을 통해 2 차적 반월판-낭 구조의 이완을 방지하는 것이 정상 회전 안정성을 회복하는 가장 중요한 치료일것으로 사료된다.
