Clostridium difficile(이하 C.difficile)은 항생제 사용에 의해 발생하는 원내 설사의 주요 원인균이다. 본 연구에서는 환자군에서 투여 항생제군별, 투여 기간, 노출된 항생제 종류수에 따른 Clostridium difficile associated diarrhea(이하 CDAD)의 위험성을 후향적으로 평가하고자 하였다. 또한 CDAD를 치료하기 위한 약물 치료 현황을 살펴 보고자 하였다. 본 연구에 포함된 환자는 2007년 1월 1일부터 12월 31일까지 C. difficile toxin assay 검사 처방이 의뢰된 환자 중 양성 42명, 음성 87명으로 총 129명이었다. 항생제 사용에 따른 CDAD의 발생 영향을 평가하기 위해 투여 항생제를 16개군으로 나누고 각각 항생제의 투여 유무를 살펴보았으며, 주요 위험군 항생제인 3세대 cephalosporin, aminoglycoside, fluoroquinolone은 8일 미만, 15일 미만, 15일 이상으로 투여 기간을 조사하였다. 또한 노출된 항생제의 종류수를 조사하였다.투여된 16개 분류 항생제군 중 4세대 cephalosporin은 C.difficile의 위험성을 증가시켰고(OR 5.96, p=0.017), metronidazole은 위험성을 감소시켰다(OR 0.30, p=0.009).투여 기간과 CDAD와의 위험성을 살펴보면 3세대 cephalosporin(OR=3.81, p=0.037)과 aminoglycoside(OR=5.50, p=0.013)는 8일 미만으로 투여한 환자군보다 15일 이상 투여한 환자군에서 CDAD의 위험성이 증가하는 것으로 나타났으며 이는 통계적으로 유의하였다.투여 항생제 종류수는 양성인 환자에서 2가지 종류 이하의 항생제를 투여하는 환자 비율이 45.2%로 음성 환자 21.8%보다 높은 것으로 나타났다.C.difficile toxin assay 양성인 환자의 평균 치료 기간은 13.24±7.19일로 60.5% 환자가 10~14일의 치료 기간을 유지하고 있었다.본 연구에서 CDAD와 투여 항생제 사이의 위험성은 존재하였으며, cephalosporin, aminoglycoside는 투여 기간이 길어질수록 CDAD의 위험성이 커지는 것으로 나타났다. 이 연구는 대상 환자수가 적고, 후향적 연구의 한계점을 가지므로 이를 보완할 대규모 심화 연구를 통해 CDAD의 event를 감소시키기 위한 노력이 필요할 것이다.
Background : Clostridium difficile is the the primary reason of the nosocomial diarrhea. The antimicrobial therapy plays a central role in the pathogenesis of Clostridium difficile associated diarrhea(이하 CDAD). Although nearly all antimicrobial classes have been associated with CDAD, clindamycin and third-generation cephalosporins have traditionally been considered to the greatest risk factor. Recent studies have also implicated fluoroquinolons as high-risk agents, a finding that is most likely to be related in part to increasing fluoroquinolones.Purpose : The purpose of this study was to determine the risk factors of CDAD related to the administered antibiotics, includingantibiotic classes and treatment duration.Method : We performed a retrospective case-control study in the patients with order of Clostridium difficile toxin assay at I Hospital (Incheon, South Korea) during the period from 1 January 2007 through 31 December 2007. Administrative, laboratory, and pharmacy data were collected from the Electronic Medical Databases.Results : The analysis included 129 reported C.difficile toxin assay results, with 42 positive cases and 87 negative cases. Significant antibiotic risk factors for CDAD included the use of fourth-generation cephalosporins(OR=5.96, 95% CI 1.37-25.98, p=0.017). Administration of metronidazole was protective against CDAD(OR=0.30, 95% CI 0.12-0.74, p=0.009). Prolonged antimicrobial therapy has been associated with an increased risk of CDAD. Third-generation cephalosporins(OR=3.81, 95% CI 1.08-13.41, p=0.037) and aminoglycoside(OR=5.50, 95% CI 1.43-21.10, p=0.013)demonstrated greater risk for CDAD over 15days than under 8days treatment duration.Conclusion : This study confirmed previous studies regarding the risk of acquiring CDAD. The fourth-generation cephalosporin was the significant risk factor compared with other antibiotics, whereas metronidazole appears to be protective agent.
