In vitro-in vivo correlation (IVIVC) is a mathematical model that can predict pharmacokinetics based on the dissolution profile and is used to develop sustained-release (SR) formulations. Although conventional IVIVC models have limited applicability, population pharmacokinetic (POP-PK) based IVIVC models can be applied to various drugs with complex absorption processes. Nevertheless, attempts to apply IVIVC to the development of SR formulations for drugs with active metabolites have been limited. For these drugs, bioequivalence may not be guaranteed by the pharmacokinetics of the parent drug. Moreover, it is difficult to explain and predict the pharmacokinetics of the parent drug and active metabolites simultaneously and reflect them to IVIVC. This study aimed to describe the pharmacokinetics of aceclofenac and its metabolite, diclofenac, simultaneously by applying the POP-PK model and establish IVIVC. For this study, three types of aceclofenac tablets with different release rates were prepared to evaluate in vitro dissolution and in vivo pharmacokinetics. SR formulations of aceclofenac were manufactured by the wet granulation method, and in vitro dissolution test was conducted by the paddle method. In vivo pharmacokinetics was determined in dogs following the oral administration of aceclofenac SR formulations. Plasma concentrations were determined by a validated liquid chromatography-tandem mass spectrometry method for simultaneous quantification of aceclofenac and diclofenac. POP-PK model of aceclofenac with the active metabolite was developed based on the obtained plasma concentration-time curve. The absorption of aceclofenac was described by incorporating a time-dependent absorbed fraction and transit compartments. Then, diclofenac was produced from the metabolism of aceclofenac in the central compartment. The developed POP-PK model could adequately describe all the obtained blood concentration profiles of aceclofenac and diclofenac. Then, in vivo dissolution was estimated by this POP-PK model, and the correlation with in vitro dissolution was derived, establishing the final IVIVC model. The IVIVC model was utilized to predict in vivo pharmacokinetics of both aceclofenac and diclofenac from in vitro dissolution of aceclofenac SR formulations. The prediction error of maximum plasma concentration (Cmax) and area under the plasma concentration vs. time curve (AUCall) was less than 11.56% for aceclofenac and 11.07% for diclofenac, satisfying the FDA criteria. This population pharmacokinetics-IVIVC model may be helpful in the formulation development of drugs with active metabolites.
In vitro-in vivo correlation (IVIVC)은 용출프로파일로부터 약물동태를 예측할 수 있는 수학적 모델로 서방정 개발에 사용되는 도구이다. 기존의 IVIVC 모델은 투과성이 높은 약물에만 적용되는 한계를 갖고 있지만, 집단 약동학적 모델에 기반한 IVIVC 모델은 복잡한 흡수과정을 갖는 다양한 약물에 적용할 수 있다. 그럼에도 불구하고, 활성대사체를 갖는 약물의 서방정 개발에 IVIVC 를 적용하려는 시도는 제한적이었다. 이들 약물에 대해서는 모약물의 약물동태로 부터 약효의 동등성을 보증할 수 없으며, 기존의 방법으로 모약물과 활성 대사체의 약물동태를 동시에 설명하고 예측하여 IVIVC 에 반영하는 것이 어렵기 때문이다. 따라서, 본 연구는 집단 약동학 모델을 이용하여 아세클로페낙과 활성 대사체인 디클로페낙의 약물동태를 동시에 설명하고 이를 기반으로 IVIVC 모델을 확립하는 것을 목적으로 하였다. 이를 위해 방출속도가 다른 3 종의 아세클로페낙 서방정을 제조하여 용출과 약물동태를 평가하였다. 아세클로페낙 서방정은 습식과립법을 이용하여 제조하였으며, in vitro 용출은 패들법을 사용하였다. In vivo 약물동태 평가를 위해, 아세클로페낙 서방정을 비글견에 경구투여한 후 아세클로페낙과 그 활성 대사체인 디클로페낙의 혈중농도를 LC-MS/MS 분석법을 이용하여 동시 정량하였다. 얻어진 아세클로페낙과 디클로페낙의 혈중농도-시간 곡선을 바탕으로 아세클로페낙과 대사체의 약물동태를 설명할 수 있는 집단 약동학 모델을 개발하였다. 약물의 흡수부위를 고려하여 시간에 따른 약물흡수율을 다르게 하였으며, 이후 방출된 약물이 transit 컴파트먼트를 단계적으로 거쳐 체순환 컴파트먼트로 흡수되는 것으로 나타내었다. 디클로페낙은 아세클로페낙의 대사로부터 생성되는 것으로 나타내었다. 개발된 집단 약동학 모델은 아세클로페낙과 디클로페낙의 모든 혈중농도 프로파일을 적절하게 설명할 수 있었다. 이후 이 집단 약동학 모델을 통해 in vivo 약물의 방출을 예측하고 in vitro 용출과의 상관관계를 도출하여 최종적으로 IVIVC 모델을 확립하였다. 개발된 IVIVC 모델은 아세클로페낙서방정의 in vitro 용출 프로파일로부터 in vivo 혈중농도-시간 곡선을 예측함으로써 검증하였다. 최종 모델의 검증을 통해 확인된 아세클로페낙의 최고혈중농도(Cmax)와 혈중농도-시간 곡선하면적(AUCall)에 대한 예측오차는 11.56%와 9.82%이내였으며, 디클로페낙의 Cmax 와 AUCall 에 대한 예측오차는 각각 10.86%와 11.07%이내로 모두 FDA 기준에 부합하였다. 본 연구에서 개발된 집단 약동학모델 기반 IVIVC 는 활성대사체를 갖는 약물의 서방정 개발과정에서 원하는 약물동태를 나타내는 제제의 설계와 개발에 유용하게 이용될 수 있다.
