의학 그리고 의료관련 전문직에 대한 교육은 의사가 교수자로서 가르치기에는 너무나 방대한 내용을 포괄하고 있다. 따라서 의사인 교수자로서 할 수 있는 일은 의료 현장에서 부딪히는 실질적인 문제 상황, 곧 임상적 문제 상황을 교육환경으로 바꾸어 학습자들에게 제공하고, 학습자들은 실제적 교육환경 속에서 자기주도적 참여를 통해 주체적으로 학습을 이끌어가도록 교육시키는 일 것이다.이러한 변화는 의료 환경의 변화와 의료 정보의 공론화에 따라 의과대학에도 변화가 필요하고, 단순 진료자로서의 의사보다는 사회에 적극적으로 참여를 요구하는 의학자로서의 변신을 요구하는 사회적 요구와 맥을 같이 할 것이다. 한마디로, 대대적인 의학교육 패러다임의 변화는 필연적이라고 할 수 있겠다.이러한 변화의 강한 요구에 입각하여, 본 연구에서는 디지털시대에 적합한 새로운 의학교육 방식을 응급의학 교과환경의 재구성이라는 틀로서 새롭게 조명해보았다. 곧, 21세기 지역사회에서 원하는 전문 의료인을 양성함에 있어 의과대학/의학전문대학원에서 설정한 교육목표와 세부 과목인 응급의학과 교과과목을 구성주의 교육 철학에 입각하여 재구성함으로써 과학적 근거와 인문적 소양을 갖춘 의사 양성의 방향성을 제안해보고자 하였다.특히 디지털 시대라는 명칭에 걸맞게, IT를 의학교육환경의 핵심적 학습도구로 설정하고, 그 환경의 구체적 이론적, 실천적 근거로서 구성주의라는 학습자 중심적 학습이론을 바탕으로 논의를 전개하였다. 구성주의적 학습자 중심의 학습이론의 대표적 모형으로는 PBL이 있다. 그리고 이 PBL은 현재 의과대학의 대표적인 개혁적 학습모형으로 적용되고 있다.그러나 본 연구는 의과대학에서 이루어지고 있는 대부분의 PBL수업과 달리, 다양한 IT를 PBL과 연계한 수업사례를 실시하였다. 곧, 첫 번째 사례로서, 응급의학수업에서, PBL환경을 웹과 접목하여 언제, 어디서나 학습할 수 있는 환경을 제공함으로써, 의학교육의 내용적 측면의 충실한 학습결과는 물론이고, 나아가 자기주도적인 학습을 할 수 있게 하며 또한 그룹 활동을 통한 대인관계기술의 향상, 다양한 문제 상황에서의 대처 능력 등 지식기반사회에서 의사로서 갖추어야 할 지식, 태도, 자질 등을 학습할 수 있는 웹기반 PBL수업사례를 실시해 보았다.두 번째 사례로서는 ‘화상원격회의시스템(Video-Teleconference System)’을 활용한 응급의학 임상실습 PBL수업을 실시하여, 원거리에 위치하고 있는 임상 실습이 가능한 병원의 학습 현장을 연계하고 이것을 토대로 각 병원의 환자사례를 실시간으로 공유하며, 임상실습 학생들에게 현장감이 있는 환자 사례를 경험하면서 비판적 사고능력(critical thinking process)을 향상시키는 수업을 실시하였다.결국 본 연구는 현재 새로운 의학교육방식이자 환경이라고 논의되는 PBL학습환경에 IT와의 접목, 곧, 웹(web), 그리고 비디오 화상교육 시스템을 접목하여, 참된 의미에서 디지털 시대에 적합한 새로운 의학교육 환경 및 방식의 확립과 활성화를 위한 기초적 연구로서의 역할 및 자료를 제공해보고자 하였다.
The subjects and learning materials in field of medicine and its related professions are extremely vast. This creates difficult challenges for teachers/professors.For the successful training of medical doctors, professors must develop a educational program that will assist their students in real life, doctor-patient experiences. Furthermore, the education of students must instill a constant drive for self development in their field. In Korea, we can see this concept of education beginning in our medical colleges.The changes brought to our world by the easy and wide access of information from the internet has created new challenges for medical doctors. Today's patients have greater access to medical information and this can lead to conflicts of opinion with their doctors and their diagnosis. The traditional thought of physicians cannot cope with this new reality.Understanding this, I have begun to reconstruct the curriculum for emergency medicine in my department. The new curriculum will focus upon preparing a physician to work in the digital era. This study is based upon the education goals of our medical college and the philosophy of constructivism. The central goal is to create balanced medical physicians who have a background in medical science and the humanities.Constructivism may be considered an epistemology (a philosophical framework or theory of learning which argues humans construct meaning from current knowledge structures). These arguments about the nature of human learning guide constructivist learning theories and teaching methods of education. Constructivism values developmental (appropriate facilitator-supported) learning that is initiated and directed by the learner. This is the path through which educators wish to approach students in constructing meaning of new concepts.According to the model of constructivism, medical professors must develop a learning environment that encourages students' active participation in all areas of their study. This environment must include authentic experience in their learning tasks. For this reason, I chose emergency medicine as an ideal setting for the application of this theory. Emergency Medicine provides an environment where collective critical thinking is possible and real practical skills can be developed.The use of Problem Based Learning (PBL) activities connected with world wide web provides students with limitless opportunities. This ubiquitous environment gives students access to a huge amount of information, opportunities for interactive discussion and the chance to develop stronger communication skills.The study calls for the application of a video teleconference system linking all of our hospitals. It will allow students access to a wider range of experience and knowledge in real time, no matter where they are located. This system gives the students greater motivation to be critical thinkers and gives them the sense that they are gaining real experience.
