현재 결핵 치료제로 사용하고 있는 약제로는 1차 약제인 isoniazid (INH)와 pyrazinamide (PZA)를 포함하여 혼합처방을 하고 있다. 이 같은 항결핵제는 치료 동안에 결핵균에 직접 작용하여 균을 사멸한다고 알려져 있으며, 숙주세포 내에 어떤 면역반응을 유도하는지에 대하여는 아직 연구된 바가 없다. 본 연구에서는 결핵균이 감염된 세포에 항결핵제를 처리하였을 때 숙주세포 내에 유도되는 선천면역반응을 확인하고, 유도된 면역반응이 숙주세포에 어떤 영향을 미치는지 연구하고자 하였다. 항결핵제는 결핵균이 감염된 대식세포에서 자가포식의 활성화를 유도시켰고, 자가포식소체와 리소좀과의 결합을 증가시켰다. 또한 결핵균 감염세포에 INH 또는 PZA를 처리하면 박테리아 유래의 활성산소족 (Reactive oxygen species, ROS)에 의해 강력하게 세포 내와 미토콘드리아 유래의 활성산소를 증가시켰고, 자가포식의 활성화를 촉진하였다. 흥미롭게도 항결핵제에 의해 박테리아 유래의 활성산소 (hydroxyl radical)가 생성되고, 이는 숙주세포 내에서 활성산소를 촉진하며, 자가포식의 활성화를 촉진하였다. In vivo 모델로서 초파리에 Mycobacterium marinum 을 감염시켜 항결핵제가 함유된 배지에서 자가포식 활성을 관찰한 결과, 항결핵제를 처리하면 그렇지 않은 대조군에 비해 자가포식소체와 리소좀의 결합이 촉진됨을 알 수 있었다. 더욱이 항결핵제 처리에 의한 자가포식의 활성화는 칼슘과 산화질소 (nitric oxide, NO)의 생성, AMPK 의존적으로 활성화되었으며, 박테리아 유래의 factor에 의해 자가포식이 활성화되는 것을 확인하였다. 이러한 연구 결과를 종합해 볼 때, 항결핵 치료 후 숙주세포의 자가포식 활성화가 관찰되며 이는 성공적인 항결핵 치료에 매우 중요한 역할을 할 것으로 생각된다.
Tuberculosis is one of the leading causes of death from an infectious agent, with approximately two million deaths annually. Currently, anti-tuberculosis chemotherapy is encountering numerous challenges, including co-infection with human immunodeficiency virus, adherence issues, and the emergence of multi-drug resistant strains of Mycobacterium tuberculosis. However, the majority of target-based discovery programs are unlikely to be effective. Understanding the host response of M. tuberculosis-infected macrophages may assist in the development of more effective treatments against intracellular M. tuberculosis infection. In this study, we investigated the host autophagic responses during antibiotic treatment against M. tuberculosis infection. We demonstrate that isoniazid (INH) and pyrazinamide (PZA) promote autophagy activation and phagosomal maturation in M. tuberculosis-infected macrophages. Treatment of M. tuberculosis-infected macrophages with INH or PZA caused significant activation of cellular and mitochondrial reactive oxygen species (ROS) and autophagy, which was triggered by bacterial-derived hydroxyl radical generation. LC3-II protein analysis in wild-type (WT) and NOX2-knockout (KO) BMDMs further showed that NOX2 is required for autophagy activation upon antibiotic treatment after M. tuberculosis infection. An ROS scavenger specific to mitochondrial ROS (mito-TEMPO), which effectively reduces mitochondrial ROS levels, significantly reduced INH-induced autophagy activation in M. tuberculosis-infected BMDMs. Interestingly, our data showed that supernatants from bacteria killed (BKS) from antibiotics-treated mycobacterial cultures induced autophagy in uninfected macrophages by bacterial-derived factors. Additionally, formation of autophagosome was increased in nitric oxide (NO) production, calcium and AMPK-dependent manners during anti-TB drug treatment in M. tuberculosis-infected BMDMs. Together, these studies also showed that the bacterial-derived factors by antibiotics treatment induce host cell autophagy, which is mediated through various mechanism including the production of cellular ROS, mitochondrial ROS, calcium release, AMPK activation, and NO production.
