수의학분야의 정형외과 수술에 있어 골이식에 대한 연구가 활발히 이루어지고 있다. 골이식은 종양, 외상으로 인한 골절, 심각한 감염과 골수염, 그리고 불유합으로 골 결손이 광범위하게 생길 경우, 이를 치료하기 위해 사용될 수 있다. 골이식의 가장 이상적인 재료는 자가골이나 이 방법은 충분한 양의 뼈를 채취하기에 적합하지 않다. 주된 뼈 이식의 재료는 같은 종의 동물에서 채취된 동종골이다. 동종골 이식편은 골유도능, 골전도능, 그리고 물리적인 지지를 제공한다. 그러나 동종골의 골유도능은 매우 취약하며, 혈액공급이 이루어지지 않기 때문에 임상증례에서 높은 합병증 확률이 보고된 바 있다. 이를 극복하기 위해 성장인자가 동종골 유합에 미치는 영향에 대한 연구가 활발하게 진행중이며, 동종골을 성장인자와 조합하였을 때, 동종골이 생물학적으로 활성화될 가능성을 보여주었다. BMP-2 (Bone Morphogenetic Protein-2)는 골아세포 분화에 중요한 역할을 한다. BMP-2는 여러 임상 연구에서 뼈의 재생에 좋은 결과를 보여 주었지만 단점도 가지고 있다. 생리적인 환경에서 BMP-2는 7 - 16분의 짧은 반감기를 가져 효과를 지속시키기 어렵다. BMP-2의 과도한 용량이 일시적으로 방출 될 경우, 이에 대한 부작용으로 조직 부종, 뼈의 과 성장, 원하지 않는 곳에 뼈의 형성, 종양 유발 등이 보고 되고 있다. 그렇기 때문에, 일반적으로 특정한 전달 물질부터 적절한 농도의 BMP-2를 서방형으로 방출되는 방법들이 고안되고 있으며, 기존에 과도한 용량의 BMP-2 사용으로 인한 부작용을 극복할 수 있는 잠재적인 방안으로 고려되고 있다.본 연구의 목적은 서방형으로 방출되는 BMP-2를 탑재한 낙엽적층구조를 가지는 다공성 비드가 동종골의 골유합에 미치는 영향을 평가하였다.실험견으로는 임상증상이 없고, 신체검사, 전혈구검사, 혈청화학검사, 방사선 검사를 바탕으로 건강하다고 판단되는 12마리의 개의 왼쪽 요골 골간에 임계 크기 골 결손을 만들었다. 결손부의 크기와 동일한 크기의 동종골을 이식하여, 플레이트와 스크류로 내고정하였다. BMP-2군은 BMP-2가 탑재된 낙엽적층구조 다공성 비드와 동종골을 이식하였고, 대조군은 BMP-2가 탑재되지 않은 낙엽적층구조 다공성 비드와 동종골을 이식하였다. BMP-2군은 6마리를 실험하였으며, 대조군은 6마리를 실험하여 총 12마리를 수술 직후, 술후 2, 4, 8, 12주 차에 방사선검사 및 혈청화학검사를 하였고, 12주차에는 조직검사와 RT-qPCR을 평가하였다.실험견 12마리 중 11마리에서는 합병증 없이 잘 유지되었다. 방사선 검사 결과 대조군의 결손 크기는 10.23 ± 1.04 mm이며 BMP-2군의 평균 결손 크기는 10.01 ± 0.91 mm이었다. BMP-2군의 방사선검사에서는 실험군이 대조군에 비하여 8주차에 더 빠르고 많은 가골 형성을 보였다. ALP, 인, 및 칼슘의 혈청화학 검사 결과 기준 범위 내에서 BMP-2군과 대조군 모두에서 유의한 변화가 없었다. 조직학적 검사에서 BMP-2군은 섬유화, 연골화, 골화로 진행되는 골 재생과 동종골 유합이 확인되었다. BMP-2 군의 많은 비드들은 붕괴된 구조가 골화되고 주변의 신생골이 관찰되었으나, 대조군에서는 골화와 신생골이 관찰되지 않았다. 12주차 RT-qPCR 검사에서 BMP-2군의 초기 골형성 인자인 runt-related transcription factor-2 (RUNX-2)가 높게 발현되었다. BMP-2와 후기 골형성 인자인 osteonectin (ON)과 collagen1A1 (COL1A1)은 BMP-2군과 대조군에서 유의미한 차이가 확인되지 않았다. 12주차까지 BMP-2의 단기간내 방출에 의한 부작용과 동종골에 의한 면역원성 합병증 역시 확인되지 않았다.본 연구의 결과, BMP-2를 탑재한 낙엽적층구조를 가지는 다공성 비드는 부작용 없이 조기 가골 형성을 유도하였으며, 초기 골 형성 인자를 상승시켰다. 따라서, 서방형으로 방출되는 BMP-2 비드를 통한 동종골 재건 수술은 수의학에서 광범위한 골 결손부위의 복구를 위한 수술법 중 하나로 이용될 충분한 가치가 있다고 판단된다.
Purpose: This study sought to assess whether the sustained release of bone morphogenetic protein type 2 (BMP-2) from leaf-stacked structure (LSS) porous beads would improve allograft integration without inducing side effects in a radial critical-sized bone defect canine model.Materials and methods: Critical long bone defect was created in radial diaphysis each of 12 healthy dogs; the BMP-2 group received an allograft plus leaf-stacked structure porous beads loaded with BMP-2 (n=6) and the control group received an allograft plus LSS porous beads without BMP-2 (n=6). The length of the bone defect was measured and the same length of bone graft was adjusted with locking plate. LSS porous beads were applied to medullary cavity of allograft and interface between allograft and host bones. Radiographic, serum chemistry, histological stains, and Real-time quantitative polymerase chain reaction (RT-qPCR) analysis were performed.Results: The surgery was performed on 12 left radii. All implant devices of 12 left radii were well maintained with no gross pathological changes. However, there was a dog, which was control group, the plate was not maintained until 8 weeks. In radiographic analysis, average defect size of control group was 10.23 ± 1.04 mm and BMP-2 group was 10.01 ± 0.91 mm. radiography in the experimental group showed a better callus formation compared to the control group within the first 8 weeks after surgery. In serum chemistry analysis of phosphorus-inorganic, alkaline phosphatase and calcium, there were no significant change in both BMP-2 group and control group within reference ranges. In histological analysis, BMP-2 group showed fibrosis, chondrification and ossification of progressing bone regeneration. Allograft integration is observed in BMP-2 group. Ossification and new-bone formation were observed around beads in the BMP-2 group, while not observed in the control group. In RT-qPCR, runt-related transcription factor 2 (Runx-2) of BMP-2 group is increased compared to that of the control group within the 12 weeks after surgery, which is a key transcription factor associated with osteoblast differentiation.Conclusion: Sustained release of BMP-2 from LSS porous beads induce early periosteal callus formation and higher expression of RUNX-2, which is a key transcription factor associated with osteoblast differentiation. These findings indicate that BMP-2 added to bone allograft inducing early graft incorporation without negative effects in clinical outcome.
